Cooperative Work Experience

Memorandum of Understanding

1. I, Carlito D. Rocafort of the Department of Education, SOCCSKSARGEN Region,

permits Kurt Ryan P. Jesura of Notre Dame of Marbel University to undergo Student
Training Program in our agency/office according to the requirements of her school for the
purpose of gaining practical knowledge and experience as student trainee from February
15, 2022 – April 30, 2022.

2. The program of training is designed to run for a maximum of 100 hours in practical work
experience.

3. The training will be in accordance with the general outline arranged for STUDENT
TRAINING PROGRAM.

4. The status between the student and placement coordinator while in training shall be that of a
student trainee-trainor and not of an employee-employer relationship.

5. The student trainee must devote a minimum of five (5) hours and a maximum of eight (8)
hours a day to the work and be constantly under the supervision of the training agency and
the placement coordinator of the department head concerned, depending upon the academic
schedule of the student-trainee.

6. While in training, the student shall progress from job to job in order to gain experience in
all phases of operation and duties until he/she becomes proficient enough.

7. The student trainee shall observe the rules and regulations of the training agency and to
abide with all implied and stated terms in the memorandum.

8. The student agrees further to extend every effort to comply with all the requirements and to
notify the agency and the school department head concerned, of any unavoidable absences
from training.

9. The training agency shall make a report to the department head through its coordinator, the
progress of the student trainee as to his/her work attitude and regularity of attendance.

10. The Waiver of Claims, Duties, and Responsibilities of the school and agency shall become
part and parcel of this MEMORANDUM OF UNDERSTANDING.

IN WITNESS WHEREOF, the parties have hereunto affixed their signatures in

Koronadal City, South Cotabato this ____ day of February 2022.
