I, JESSE OLIVA III, Filipino, of legal age, single and a resident of No. 01 Chapiz
Village, Marcos Highway Baguio City, after having been duly sworn in accordance with law,
hereby depose and state:

1. That I was issued a School Identification Card by the University of the Baguio City;

2. That said Identification Card is valid up to the present time;

3. That the said Identification card was lost when my wallet containing the same was
lost while I was shopping at the SM City Baguio yesterday;

4. That despite diligent efforts to find the same, it was not found;

5. That the same was not confiscated by the authorities for violation of school rules and
regulations;

6. That the same is now beyond recovery;

7. That I am executing this affidavit to attest to the truth of all the foregoing.
