Weekly Internship Journal Report

From the on-boarding date onwards, the student trainee will be required to submit a
weekly journal report enumerating his/her daily activities to the Student Internship
Coordinator at the Human Resource Department. This report should be a clear, concise, and
accurate account of all the types of activities that the intern participated in and/or observed.

During the week, interns should make notes of their observations, thoughts, insights,
and feelings. At the end of five working days, they should submit their daily observations
being certain to leave out any names, situations, or activities that are confidential. The weekly
activity report should be duly signed by the intern and his/her immediate supervisor and
submitted to the student internship coordinator every Monday of the succeeding weeks for
confirmation and approval.

At the end of the internship proper, all weekly reports shall be submitted to the student
internship coordinator and student-trainees will be asked to accomplish a post-internship
feedback report.
