Name of agency and why you selected this agency

I chose Banner Health Cardiac Rehab. I picked this organization because I have worked in this

agency before and I had a good experience there. I was a full-time worker at cardiovascular

recovery for quite a long time, and since changing to library nursing, I can in any case help in the

office on a case by case basis.

Brief history and mission of the agency

The benefits of cardiac rehab were first discovered in the early 1950s. The mission of Banner

Health Cardiac rehab is to help patients live a healthier life, it helps patients to improve their

heart’s performance through educating them, providing a healthy diet, and also through exercise.

The mission includes improving overall health and preventing future cardiac events by making

lifestyle changes learned in the cardiac rehab program (“Rehabilitation” 2021).

Target population the agency serves

The target population of Banner Health Cardiac Rehab is any person who has had a

cardiovascular event or procedure and those who may be at risk for cardiovascular disease. The

agency works closely with physicians both in the hospital and in the outpatient setting to identify

individuals that would benefit from the services provided by cardiac rehab (“Rehabilitation”

2021).

Services and programs offered

The agency provides three phases of treatment. Phase 1 is the inpatient program for those in the

hospital and qualifies for cardiac rehab. During phase 1, patients and their families learn more

about their disease process, the benefits of cardiac rehab, and important information about their
recovery after discharge. The phase 2 program is the outpatient cardiac rehab program where

patients participate in monitored exercise and education classes regarding medications, diet,

smoking cessation, and lifestyle modification. The phase 3 program allows patients to continue

participating in supervised exercise while transitioning from the phase 2 program and developing

their exercise program outside of the agency.

What criteria (i.e. income, age, etc.) must clients meet in order to be served by the agency

One of the criteria that a patient must meet to qualify for cardiac rehab is over the age of 18.

They must also have a qualifying diagnosis as mentioned above and a physician’s order to

participate in cardiac rehab. The program is physician-supervised, so the patient must have an

attending physician refer them to the cardiac rehab program to monitor their safety and progress.

Many insurance plans help cover some or all of the cost of cardiac rehab, but the agency also

offers private pay options for those who may not have insurance (“Rehabilitation” 2021).

What you found interesting about this agency or helpful for you as a professional nurse

When I first started working at cardiac rehab, I did not even know it existed. When I worked in

the hospital, no one had educated me on the program and what it could do for my patients. Once

I began working in the cardiac rehab department, I quickly learned how crucial cardiac rehab

was to the recovery of patients who had suffered a cardiac event or those at risk for heart disease.

Cardiac rehab needs to be promoted and discussed with all who may benefit. I have seen so

much success in the recovery of patients. Cardiac rehab not only helps patients recover faster,

but it helps to ease their fears of "pushing” themselves when it comes to exercise during their

recovery. I continue to promote and advocate cardiac rehab to all of my patients in the hospital

that meet the criteria ("Rehabilitation” 2021).
