SUBJECTIVE OBJECTIVE ASSESSMENT PLAN

 Epistaxis secondary to inflamed N/A Allergic Rhinitis (Hay fever) Drug Parameter:
nasal mucosa
Ceterizine (Zyrtec) 5 to 10 mg orally
once a day
MOA:
works by blocking histamine (H-1)
receptors on cells.

Fluticasone Propionate
(Fluticasone) Nasal Spray, 2 sprays
(50 mcg of fluticasone propionate
each) in each nostril once daily (total
daily dose, 200 mcg).
MOA:
Fluticasone binds and activates
glucocorticoid receptor, resulting in
the activation of lipocortin.
Lipocortin, in turn, inhibits cytosolic
phospholipase A2, which triggers a
cascade of reactions involved in the
synthesis of inflammatory mediators,
such as prostaglandins and
leukotrienes.

Non-drug Parameter:
 Nasal tampon for epistaxis
 Avoid going outdoors during
periods of high pollen counts
 Ginger tea to alleviate nasal
congestion and headaches
