HOUSEKEEPING

ACCIDENT/INCIDENT REPORT

Regarding the accident that happened at around 2:00 pm Friday February 25, 2021
at KMC Armstrong Phase 2 fit-out Project.
Mr. Rico Berto Ariola (Admin Installer) while he drilling for installation of cabinets at
Penthouse accidentally something got to his right eye. The accident happened 2pm
of Feb. 25, 2022 and he reported 8am of Feb. 26, 2022.

Investigation:
- His not wearing eye protection/safety goggles
- He didn’t report immediately the accident happened to him.
- He undergoes to self-medication.

Action Taken:
- because he was too late to report that accident, the situation of his right eye
has worsened.
- Immediately rush to the nearest hospital

Recommendation:
- all workers mandatory to wear safety goggles and face shield while they’re
having a Grinding and Drilling activity.
Under Rule 1080 of Occupational Safety and Health Standard and
Under Section 6 of Department no.13.

- Remind all workers to wear appropriate PPE for their working activity.
