Action Plan

Goal:
The goal of my project is to ensure that every student has a good relationship with their counselor to help create a positive high
school experience.
Objectives:
1. The first objective of my action plan is to have students meet with their assigned counselors to talk about their goals,
schedule, plans after high school, etc.

2. The second objective of my action plan is to have the students who feel like they need a different counselor fill out a
counselor request form within the first four weeks of school.

3. After being approved by the administration, students who requested a counselor change will be able to meet with their new
counselor.
