A contractor's employee hit the trolley with waste conveyor

Narrative/ Description:

On above date & time, an employee of M/S Batkhela Company was reversing the tractor
trolley near waste conveyor to collect the FF line waste as driver of tractor trolley was not
available at site. During reversing trolley hit with waste conveyor and resulted a
misalignment in waste conveyor. Immediately, mechanical team aligned the conveyor and
get it ready for operation.

Findings:
1) Carelessness of contractor' s employee as he was not authorized to drive the tractor
trolley inside plant premises
Basic Cause:
