Narrative/ Description:
On above date & time, washing labor person(Usman) observed sparking at pea-podder emergency
switch cable during washing activities at area, so he informed process operator and after check burnt
cable was observed.
Immediately after incident reporting main breaker was turned off and E&I technician was informed
and he disconnected terminals. Later on cable was replaced

Findings:
1) Sharp edge/ corner of cable tray damaged insulation of cable

Basic Cause:
1) Sharp edge

Recommendations: Responsibility Target date

1) Visual inspection of all cables in pea podder area

should be carried out M. Tariq Immediate

2) Sharp edges on cables trays should be removed after
