Narrative/ Description:
On above mentioned date and time, Lifter operator of M/S Al-Razzaq was clearing access for walk
ways in cold stores area, when he was picking up pallet, pallets skidded due to wet surfaces/Ice and hit
on foot of worker who was standing nearby. It caused minor pressed on ankle of worker.
Painkiller gel was applied on affected area and he resumed duty onwards.

Findings:
1) Negligence by lifter operator and worker as they did not maintain 5 feet distance between lifter and
worker when picking pallet.
