Narrative/ Description:
On above mentioned date and time, , a worker of M/S HRSG Services and management trainee officer
(MTO) were going from plant area to colony, when they reached near quarter no.6 of staff side, bike
stand hit with one of road brick and bike unbalanced and they both fell down. As a result, driver's
knee hit with road and caused a scratches, no injury to MTO.
During investigation, following were observed
1) Bike driver did not release the bike stand properly at its exact position before driving.
2) Driver choose wrong road for going to colony as it was not levelled and administration pasted
warning sign for not using this path for plant entry/exit.
3) Driver did not had driving license.
After injury, first aid was given to bike rider and sent him district head quarter hospital (DHQ) for
further treatment, after some treatment at DHQ, he was discharged.
Findings:
1) Negligence by bike rider as he did not release bike stand, and chose wrong path/road for coming at
plant
2) Uneven road
3) Driver was not authorized to driver due to non availability of driving license.
Basic Cause:
1) Negligence 2)Uneven road 3)Driving without driving license.
Recommendations: Responsibility Target date
1) Only authorized person (having driving license)
should be allowed to bike/vehicle at plant, Ahmad Fawad Immediate
