In addition, the Contractor complies with the quality management of construction works,
responsibilities of construction contractors and relevant articles in the Construction Law, Bidding
Law and the Decree on Construction Quality Management. .
2.2 Supplies and materials.
2.2.1 Technical specifications, origin and standards applicable to main supplies, materials and
equipment used for the work.
Based on the technical criteria and requirements of the Bidding Documents, Design Documents,
the main supplies and materials will be submitted samples together with the material certificates and
approved by the Investor and the Consultant before putting into use.
2.2.2 The main building materials.

Cement: Cement used for construction is mixed Portland according to cement standards
TCVN 6260:2009; Portland cement – Technical requirements TCVN 2682:2009.
The Bidder must at all times provide certification from the cement manufacturer to the
required standards for the period of use, which is issued and signed by an independent consultant.
Each batch of cement must be accompanied by a manufacturer's certificate indicating that the
cement has been tested and analyzed for its chemical composition and physical properties. Each
batch of cement after being brought to the site will be tested and reanalyzed according to current
standards.
Cement should be kept at the site in the right conditions.
Hot cement needs to be stored, not used immediately but not longer than 28 days at the site
warehouse. Do not use cement that has been produced for more than 12 months.
Cement must be delivered in the form of intact packaging and labels on the bags, cement
must have enough at the site to ensure continuous construction and must be used in the shortest
time and rotated to avoid the case of storage for a long time, causing poor quality. Cement
transferred to the construction site must be well preserved to prevent water infiltration and
humidity due to climate. Cement stacked in warehouses must have a platform of 30cm above the
ground, not more than 09 bags. It must be well ventilated and moisture resistant. Cement before
use must be checked the quality. Any cement stored at the Site that does not conform to the current
technical standards and quality standards, the Bidder must remove from the Site.

Sand: Must be supplied from a specified source and in accordance with current technical
standards and must be washed when used, ensuring the correct distribution according to design.
Sand must be obtained from a source capable of providing sand of good quality regularly and
reliably throughout the construction process.
Stone: Must be supplied from a specified source in accordance with the standards "Complete
reinforced concrete structures", "Aggregate for concrete and mortar - Technical requirements
TCVN-7570: 2006".
Needs to be hard, durable, clean, not dirty by impurities that affect the strength and
durability of concrete, such as clay particles, mica particles, coal, organic impurities, iron ore,
sulphate salt, etc. calcium, magnesium. Aggregates must not be mixed with mollusc shells.
Inspector must check the supply of tests and requirements for materials before putting into
construction. Aggregates lacking test results are not used.
Commercial concrete: A certificate of quality and origin at the place of manufacture is
required. The technical requirements comply with the requirements of the Design. Before
construction, the Bidder must submit the distribution and source of input materials.
Steel: The steel put into use must conform to design requirements. Limit the change of type
compared to the design documents during construction, especially do not replace high strength
steel with lower intensity steel. All bearing steels must comply with the standard "Reinforced
concrete structures TCVN 5574: 2012" and "Concrete reinforcement - TCVN 1651-2008". No
rust, no oil or dirt. Steel must meet ISO 9001 standard.
When using imported steel, a technical certificate must be attached and it is necessary to take
test samples according to the standard "Metallic materials - Tensile test - Part 1: Test methods at
room temperature TCVN 197-1:2014" and "Metallic materials - Bending test TCVN 198-2008".
Bolts: High-strength bolts (including nuts, washers) after being processed, before being put
into use, must be checked by the technical inspection department of the investor and tested in
qualified laboratories. If it passes, it will be tested and put into use.
Bolts for the main structure have a strength of 8.8; yield limit 640; breaking strength 800.
Bolts for substructure and anchor bolts have a strength of 5.6; yield limit 300; breaking
strength 500.
Water: Must take samples for analysis and conform to the standards of water regulations
according to the standard TCVN 4506-2012.
Tiles, paving stones of all kinds: Tiles and paving stones of all kinds must be of the right
type and size according to the design, ensuring quality without warping and must have a clear
origin;
Seam-lock roofing sheet, thickness 0.53, AZ150
EPS panel, compact partition, plaster ceiling, paint, door materials...: Must be of the right
type, meet the technical requirements of the design, have a clear origin.
Painting supplies, electricity, water, lightning protection...etc: Requires the right type, must
have a certificate of quality and origin at the place of production. The technical requirements
comply with the requirements of the Design.

In order to prevent and limit errors that occur during the construction process, we use the
method of inspection and review right from the first stage. For materials and construction

meeting design requirements, and be brought to internal inspection first, then the Investor,
Supervision consultants will check. All types of materials and equipment, when brought to the site
for installation, are approved by the Investor, an authorized person or a supervisory engineer to
check the quality and design, to satisfy the requirements of the origin, manufacturer, and

requirements stated in the Investor's Bidding Documents for the works. Materials and equipment

All equipment has a certificate of origin, the manufacturer, the manufacturer has all the

For construction equipment, it is also periodically inspected and tested by competent
authorities. For people, the Bidder realizes that this is a very important issue in ensuring the
quality of the building. We bring into the project from the Project Manager to highly qualified

All kinds of supplies and equipment are stored in the warehouse and scientifically arranged
in rows to distinguish them by number, diameter, length and code for easy taking out, use and
storage. Do not mix rusted and unrusted reinforcement. When placing reinforcement in the yard
outside the field, put 1 high end, 1 low end on a hard foundation with no grass growing. Steel
millet is at least 30 cm higher than the foundation, not more than 1.2 m high and 2 m wide.
Cement is stored in the warehouse according to the prescribed standards to prevent water
penetration and moisture due to environmental climate. The storage time is guaranteed not to
exceed the specified period. The cement bags that have had the phenomenon of clumping are
absolutely not put and used. Cement must not clump, the amount of cement passing through sieve
No.008 is greater than 85% of the total weight. Cement is kept at the site in a condition that does
not alter its quality. Sealed, non-tear, puncture-proof cement bags are placed above water and
ventilated on a floor more than 30cm above the ground and protected from water. Loose materials
must also be poured in a clean, dry place with separation to avoid contamination of materials by
mud, grease, etc. Electrical supplies, compact partition panels, gypsum ceiling panels... are stored
in a closed, dry and cool warehouse to ensure that weather factors do not affect the quality of the
materials.
The construction site has from 1 to 2 storekeepers who are responsible for importing and
exporting materials daily. Storekeepers are responsible for making inventory cards in daily books,
regularly checking warehouses and yards to promptly handle problems that may occur during the
construction process.
Site security is responsible for standing 24/24h at the construction site to protect security and
order on the construction site to avoid loss and damage of materials.
Inspection and supervision of material quality, construction work is carried out both in the
field and in the laboratory using advanced tools and equipment to assess the quality of building
materials. We put in the works good engineers, experienced in construction and supervision. A
team of skilled workers has executed many projects and works with high quality.

- To operate the project, the Contractor establishes a Project Management Unit. Under the
executive board are the assisting department and direct construction teams, performing the construction
of specific items of the work.
Relationship between the Board of Management and the site management department
- The Board of Directors of the Company directly guides the departments and the Site
Management Unit, assigns departments in the field of expertise, directly guides and inspects the site
manager to effectively execute the contract, take appropriate construction methods, processes,
regulations, quality acceptance, technical handling, equipment and construction safety, supply of
materials and human resources.
- The site management department, which is directly the site manager, must regularly report to the
Board of Directors on the construction situation and management aspects on the site. The Site Manager
is responsible for regularly coordinating with relevant departments to fully implement the requirements.
Ensure the construction works continuously according to the Design documents to ensure the technical
quality of the project, at the same time coordinate to solve problems in each construction area in th best
timely and effective manner.
Site manager
- Perform the tasks assigned by the Board of Directors and take responsibility for the organization
of human resources, techniques, quality and progress of the project, check and urge daily on the
construction process, ensure the right design. Coordinate with the Owner, design consultant, Owner’s
engineer and related parties on issues in the construction site.
Site deputy manager
- Perform the tasks assigned by the site manager, to act on behalf of the site manager in charge of
all work of the construction site in the absence of the site manager.
Department of Progress, Technical and Quality Management
-The head of this department is the construction technical manager - responsible to the Site
Management Unit for the technique, progress and quality of the work.
- The technical department is directly in charge of the progress, construction techniques, quality of
each job, assisting the Site manager and technicians who are always present at the construction site, and
the relationship with Party A and the engineer, consulting engineers in construction technical solutions,
doing the acceptance work, making as-built documents, and daily monitoring diary.
Safety, security and environment management department
- Gather, research and disseminate legal documents on labor protection work to teams. Organize
training courses on labor protection for employees throughout the construction site. The safety
department is managed by a full-time officer and part-time site technicians.
- As officers are directly in charge of safety, security and environment on the construction site and
around, assisting the Site Manager and technicians, always present at the construction site. Take
measures to ensure the safety of people and equipment and ensure a good working environment.
- Responsible for taking care of the health of staff and construction workers. Dispensing drugs for

minor illnesses. When an accident occurs, the medical department will conduct first aid, give first aid to
the patient, and depending on the case, they will transfer to the hospital for emergency. Report regularly
to the site Management Unit, the company's board of directors on the situation of occupational accidents
in the construction site.
Administration and finance department
As the department responsible for monitoring and supervising the use of people, such as paying
-
workers, providing money for the material and equipment department, this department is closely related
to the direct construction teams and equipment parts.
Responsible for the supply of materials and equipment reported by the technical management
-
department, this department basically determines the quality of the product, this department is one of
the important factors determining the construction progress.
Production teams
- Follow the direction of the technical management department for each work area.
Includes teams: Construction motorbike, formwork + scaffolding + reinforcement + concrete,
-
masonry + finishing, mechanical group, aluminum and glass, plaster + paint, electricity and water team,
construction team.
Each team will include 01 team leader and 01 supervisor who is responsible for managing the
-
team's human resources, progress, and quality before the project Management Unit.
The above field organization model always puts the quality and construction progress on top. It
-
shows the scaled work and planned organization, there is the expertise of each production team, on the
other hand it ensures the dynamism of each production team.
Workforce management
- Well perform the registration of temporary absence with local authorities.
- Ensure absolute security and order, social safety in the area

6. Lean concrete work, cut the pile head, place concrete at the pile head

Clean the bottom of the foundation pit and the top of the centrifugal piles
Fabrication and erection of steel bars connecting centrifugal piles to the foundation
Carrying out placing concrete at the top of centrifugal piles (Pile head concrete is poured
manually)
7. Technical method for backfilling – compaction
- Backfilling can only be carried out after concrete pouring, foundation wall bracing, underground
pipes installation have been completed and accepted by the Owner and allows moving to the next
construction step.
- Backfilling is carried out in the following order:
Check the area to be filled, remove all excess materials and waste of previous construction
stages such as construction debris...
Mark the level of foundation to be filled with red paint on the foundation or wall
Level the areas to be filled to create a relatively flat and compact
Spread the soil in layers with a thickness of 35-40cm. During spreading, it is necessary to
remove impurities and unsuitable blocks of material. After compacting each layer to the design
thickness of 30cm

Use Mikasa tamping rammer to compact. The turns of compaction must overlap at least
10cm. The compaction process can be considered to add water to achieve optimum moisture
content.
Density check must be done regularly and deal with unsatisfactory areas.
After completing the compaction for the first layer, spreading and compacting for the next
layers can be carried out.
The process of backfilling and compacting must ensure that the compaction coefficient is not
lower than 0.95.

Formwork and brace, support system are designed in advance and will be fabricated according to
drawings. The Contractor is responsible for the structural design and dismantling of the formwork.
Defective, improperly cleaned and unsuitable formwork shall not be used. Warped, damaged, broken
or defective formwork will be removed out of the construction site
The formwork for beams and floors is designed with a curvature according to the formula
f=3L/1000. The bearing parts of the falsework are not connected. The braces are calculated and
arranged appropriately to ensure the entire system of formwork, falsework is stable.
Formwork blocking the end of pouring blocks ensures tightness and stablity, ensures the shape
and size of the stop circuit as specified by the design.
Before placing concrete, the concrete surface will be cleaned with a water pump.

Formwork must be tightly joined so as not to lose cement water
Inspection requirements for installed formwork
The installed formwork needs to be checked according to the following requirements and the
deviation must not exceed the allowable value.

design position
- Rain cover will be prepared in case it rains
Concrete work includes: Preparation of materials for concrete; Determine the mix composition
for each concrete grade and batch.
Mixing concrete
Transporting concrete from the place of mixing to the place of pouring
The concrete drop height should not exceed 1.5m -2m to avoid the case of concrete
stratification (applicable according to standard 4453-2012). If the concrete drop height is higher
than 1.5m, the contractor will take measures to reduce the concrete drop height such as adding
auxiliary chutes, moving the standing position , supporting pouring by excavator...

- HCA Contractor shall organize adequate and competent HSE Field Supervision by appointing
Site HSE Manager, Superintendents, Supervisors and Officers in sufficient quantity according to the
number of direct work employees to be supervised as well as the number of work locations to be
supervised.
- Each isolated area will be arranged with at least one safety officer per a working team
- One full time field safety officer will be arranged for per each 50 workers
- One full time field safety supervisor will be arranged for per each 200 workers
- The Vapco may varify Hsse personnel of CONTRACTOR and its subcontractors during
implementation of the Project
- The VAPCO reserves the right to reject any HCA Contractor/Subcontractor Site HSE Manager
based on continual site competency assessment. In such case, HCA Contractor will forward the new
Site H&S Manager’s CV to VAPCO within 7 days for acceptance.
1.2. Responsibilities
1.2.1. Responsibilities of Project Manager
- The Project Manager shall establish project HSE organization and appoint registered H&S
Manager as the responsible Person with defined roles, responsibility and authority to ensure that HSE
Management System is established, implemented and maintained in accordance with relevant
requirements.
- The Project Manager issues an organization chart showing the site management team executing
the Works. The project management team will be sufficient in numbers and suitably experienced and
competent to successfully perform the H&S management system.
- The Project Manager may assign responsibility for a particular job or function to a member of his
management team, and delegate the authority to enable him to perform his/her duties; not withstanding
such assignment, overall responsibility remains with the Project Manager.
- The Project Manager shall ensure that effective means of communication are put into place to
advise employees on HSE issues. He will also ensure all staff under his control undertakes training as
required by their role.
- The Project Manager shall take the lead in developing and promoting initiatives in HSE matters
and shall use the management system to establish the required standards. He shall monitor on a regular
basis the performance of the workforce under his control in achieving the HSE targets of the project.
- The Project Manager is responsible for ensuring the HSE commitment and accountability of the
project Management Team. Therefore, he has a full understanding of the fundamental objectives and
requirements of the HES policy, standards and procedures when allocating responsibilities to each grade
of supervision.
- Specifically, the Project Manager shall:
- Assure that approved HSE procedures are adopted for defining methods and systems to be
applied.
- Assure that all relevant laws, regulations and other requirements in matters of HSE are respected.
- Maintain HSE relations with Owner, Sub-contractors, Vendors and with Governmental

- Ensure that all activities regarding work safety and hygiene are carried out in accordance with
the HES plan and procedures.
1.2.3. Responsibilities of H&S Officers (Engineers & staffs)
- The H&S officers shall assist the H&S Manager by ensuring that the provisions of the H&S plan
and other H&S related requirements are correctly complied with by HCA CONTRACTOR personnel
and Sub-contractors in each working area.
- They ensure direct supervision and coordination of the Sub-contractors’ activities and day to day
contacts with them to deal with any H&S matters.
1.2.4. The H&S officers’ general responsibilities include, but not limited to:
- Verifying that all workforce under HCA CONTRACTOR’s direct or indirect responsibility
receive appropriate H&S information, induction and training, including supervision and control on the
H&S training program by Sub-contractors.
- Advising the H&S Manager of any necessary required actions to improve working and living
conditions in the field under their individual responsibility.
- The H&S officers shall attend tools box meetings
- The H&S officers shall inspect unsafe conditions and acts
- Organizing, managing and recording of meetings with sub-contractors as per the H&S relevant
meetings program.
- Ensuring adequate levels of working and living conditions by means of regular inspections on
housekeeping, hygiene, potable water system, kitchens and canteens, sewer system and waste disposal
etc.
- Leading accident, incident investigations and attending any major accident investigation in areas
under their responsibility and reporting on them to the H&S Manager as per the Accident Reporting
Procedure.
- Organizing and managing the medical and first aid facilities in areas under their control.
- Daily check and regular inspection of PPE.
- The H&S officers shall report to the H&S Manager on H&S matters.
1.2.5. Responsibilities of Construction Manager, Section Managers and Supervisors
-Construction Manager, Section Managers and supervisors generally shall assist the Project
Manager / the H&S Manager in the execution of their duties.
- Being familiar with the requirements and recommendations for Safe Working Practices Method
Statements and Risk Assessments etc. applicable to their work place and ensure that they are enforced.
- Incorporating H&S instructions into the routine day to day orders and make sure they are
understood and obeyed.
- Preventing site persons from taking unnecessary risks.
- Ensuring that all operatives/labor are suitably trained for the work in which they will be engaged
in ensuring that all-new employees, sub-contractors personnel adopt correct safety and health
precautions.

- Checking that all plant and equipment, including power and hand tools are maintained in a good
condition and remove from use any defective items.
- Maintaining organized, tidy and safe working areas.
- Maintaining all welfare facilities in a clean and safe condition.
- Ensuring that all operatives/labor are provided with and use approved personal protective
equipment.
- Ensuring that instructions are given in the use of any potentially hazardous materials.
- Ensuring that other persons including Owner representatives, other contractors and members of
the public are protected from the work activities of HCA CONTRACTOR and its subcontractors.
-Discouraging horseplay and take action against those who consistently fail to take care of their
own Health and Safety and of other persons working with them or in the vicinity.
- Stopping any activity on site if there is an immediate risk of injury to any site personnel, or the
possibility of a dangerous occurrence happening.
- Each section or working groups’ supervisors shall have a skill to monitoring, supervision and
instructions through the training courses.
1.2.6. Responsibilities of Subcontractors and Vendors Personnel.
Every Sub-contractors / Vendors Personnel working on the Project Site shall:
- Comply with all H&S rules and other requirements.
- Adhere to this H&S plan and comply with the H&S requirements and procedures. These are to
be observed as minimum requirements.
- Include in their site organization as key personnel (directly reporting to the sub-contractors site
Manager) an H&S supervisor whose function shall be to assist the site Manager in assuring that all
activities in the sub-contractors / vendors scope of work on site are carried out in accordance with
regulations in matters of H&S, the project H&S requirements, this H&S plan and safe working
procedures.
- Ensure that their own organization at Site include an adequate number (with the respect to the
total Sub-contractors’ workforce) of H&S supervisors, entitled to assist the H&S officer and enforce
suitable application of H&S procedures during the execution of the works.
- Ensure that all of their own personnel are aware of H&S requirements arising from laws,
regulations and the contract and observe them by planning appropriate training of their personnel.
- Identify and analyze all job hazards for their workforce, apply all necessary preventative and
protective measures and issue personal protective equipment to them as necessary for the work to be
performed.
- Regularly record site H&S inspections and appropriate inspections for maintenance of
installations, plant /equipment, work tools and personal protective equipment, with the objective of
identifying early any potential hazard.
- Cooperate with HCA CONTRACTOR in investigating incidents, accidents to identify and
enforce corrective actions to prevent recurrence.

- Attend H&S meetings and comply with the coordination procedures stated to prevent or
minimize risks that could possibly arise from interference between different activities being carried out
in the same areas.
2. MEDICAL PLAN
2.1. General
- HCA CONTRACTOR will provide medical facilities on Site such that medical assistance can be
rendered to workers in the event of an incident occurring. Trained first-aid personnel and suitable first-
aid equipment and facilities shall be supplied at the Site and camp. A doctor and a nurse will be
assigned at these locations. The CV of the doctor and nurse shall be submitted to Vapco for approval .
Medical resources provided by CONTRACTOR will align with the applicable standards for local
medical support services. The contractor will consider signing a medical service contract with Ky Anh
general Hospital and shall submit evidence documents to Vapco lates
- Each work Site will maintain close communication with and have access to the occupational
Medical Service (hospital or clinic).A list with details of the nearest hospital, including its location and
phone numbers, will be posted in all the Site Offices, including Subcontractors’ and the camp offices
- Prior to the start of work, arrangements will be made for medical facilities and medical personnel
to be available at the construction Site and the camp.
- Effective communication and transportation to care for injured workers will be provided.
- Where professional medical services are available, each work Site will set up terms and
conditions for use and maintain close communication with them.
- A list of the available hospitals including locations and their phone numbers will be displayed at
all Site and camp offices including Subcontractors’.
- First aid arrangements and how-to use them will be thoroughly covered in inductions and other
appropriate training.
2.2. Medical Facility
The medical facility will provide:
- A waiting / reception area, consultation / examination area, treatment area, sick bay / observation
area, sanitary facilities, secure storage for drugs and medical equipment and confidential storage of
medical records;
- A comprehensive list of medical equipment and supplies necessary to provide first aid,
immediate treatment and stabilization of major illness and injury arising out of or during the course of
the works. At a minimum this shall include a bed, stretcher, chair, table, heating, lighting, sink with
water supply, cabinets, and first aid equipment including a portable defibrillator.
- A detailed inventory of medicines and drugs shall be kept and checked regularly.
- Plans for medical transport/evacuation of injured or ill Personnel including detailed
specifications for a dedicated ambulance and arrangements for on-going care.
2.3. Medical Staff
The Contractor shall appoint at least one nurse with a college degree or higher

-Monitor the health and well-being of all Site employees and notify any adverse trends to the HSE
Manager.
- Make sure the medical facility is equipped with sufficient and adequate materials, consumables,
instruments, and staff to function effectively.
- Ensure that the medical facilities are always kept clean and hygienic and ready to use.
Be aware of any dangerous substances in use at Site, and obtain any Material Safety Data Sheets

(MSDS) to be able to take proper care when required
- Notify the HSE Manager of any trends with illness or injuries, especially for infectious disease.
- Periodically check hygiene conditions of the canteens/restaurants, including all food
- Provide all possible medical assistance and care to improve the condition of persons suffering
injury or illness.
- Be available to attend accidents/incidents on Site to give medical treatment as required.
- Carrying out training programs for construction personnel. (First aid training, etc.)
- Dispense pharmaceutical items as prescribed by the doctor and maintain records of prescription
drugs.
- Keep stock records of pharmaceutical/clinic items, and order replacement items to maintain
minimum stock levels.
- Periodically check the hygiene conditions of all camps.
- Keep accurate records of all medical matters and ensure they are kept strictly ‘Medical in
Confidence’ and restrict access to Authorized Persons only.
- Prepare a Monthly Report on major activities and topics on medical matters and monthly
statistics including trend of patients, etc..., for submitting to the HSE Manager.
- Provide guidance for HSE Manager on Incident Classifications
2.4 First Aid
- CONTRACTOR will provide the first aid equipment as per regulation of Cicular 19/2016/TT-
BYT
- CONTRACTOR will provide a room for first aid treatment that will be equipped with a bed,
stretcher, chair, table, heating, lighting, and sink with water supply, cabinets, and first aid equipment
including a portable defibrillator.
- HCA Contractor shall sign a medical service contract with Ky Anh General Hospital therefore a
24/7 first aid service and facility is available.

- CONTRACTOR and its Subcontractors will provide First Aid boxes for emergency use only.
- CONTRACTOR and its Subcontractors will supply enough First Aid boxes to suit the size of the

workforce, and locate them in close proximity to the work areas and check
-The stock of disposable medical items will be maintained according to the number of employees
and consumption.
- Guidance for medical personnel is one advanced First Aid staff per 50 workers and
- CONTRACTOR will ensure that adequate number of employees is trained in First Aid. First
- Aiders will be competent people, trained in First Aid at Work.
- First Aiders will be recognized by posted photographs, detailing name, location, and company
and contact number. First Aider’s safety helmets will have a self-adhesive green cross sticker affixed to
each side basically. CONTRACTOR Site HSE Manager will organize and control the design of the first
aid high visibility jacket or vest will be developed with other function of HSE personal identification
following BBS safety program or recognition programme.
- A first aider holding a current certificate issued by an accredited association will be able to
become “Principal First Aider” The principal First Aiders will have attended an appropriate, approved
course resulting in the award of a First Aider at Work Certificate.
- The principal First Aider will always be available at the first aid station of each Subcontractor
during the work.
-A CONTRACTOR of any incident / accident happening with the required information for the
Site H&S Statistics, as per the incident / accident reporting procedure.
The sub-contractor’s H&S supervisor shall ensure liaison between the sub-contractor and the

• For a residual risk assessment, the Risk Level is known as the Residual Risk Number (RRN).
• In both cases, the risk level dictates the response and timing of risk controls required.
Management of Risk Assessment
The management of risks identified through the risk assessment process will be determined by the risk
rating.
The amount of effort/resources that are to be committed to an identified risk is determined by the risk
score. The Contractor has a scheme of escalation.
• Risks scored 1-5: are considered low risk (L) and therefore risks are accepted (tolerated) by the
Contractor. No more countermeasures are required.
• Risks scoring 6-8: are considered moderate risks (M) and should be managed /treated so that
they are made as ‘low as reasonably practicable’. These risks will usually be managed locally. more
countermeasures are required. More actions are required to eliminate or minimize the risk using the
Hierarchy of Controls.
• Risks scoring 9-12 are considered high risks (H). Actions needed quickly (within 1-2 days).
The task should not proceed unless the risk is assessed and control options selected based on the
Hierarchy of Controls. These risks must be treated, i.e. an action plan should be developed and
implemented that seek to reduce the potential impact of the risk (i.e. reduces the risk score). These risks
will be added to the risk register and will be reviewed by the Management Committee and overseen by
the Clinical Quality, Safety, and Governance Committee High risks scoring 12 will also be reviewed by
the Board.
• Risks scoring 15-25 are considered as extreme/catastrophic risks (EH). Stop the activity, as it
is too risky to do anything else. These risks must be treated, i.e. an action plan should be developed and
implemented that seek to reduce the potential impact of the risk (i.e. reduces the risk score). These risks
will be added to the risk register and will be reviewed by the Management Committee and the Clinical
Quality, Safety, and Governance Committee High risks scoring 12 will also be reviewed by the Board.
Risk treatment plans have to be developed according to the level of risk and the needs of the
organization. In broad terms the Contractor will seek to tolerate risks (1-5), and treat risks with a score
of 6 or more, and where appropriate will seek to transfer risk to another provider, or may consider the
need to terminate the risk by terminating the aspect of service affected if no other solution can be
identified, and the risk is extreme (i.e. 15+).
