1. GENERAL OUTLINES
This Project Procedure is established for the implementation of the MARTIN LINGE Topsides EPSCC
(Engineering, Procurement, Supply, Construction, transport and Commissioning) Project.

The relevant Lump Sum and Provisional Sum Contract covers the supply of a Utility module, a Process
module made of a 3 component well bay, compression module and oil and gas processing area, a Flare
module with his flare boom, and a 95 single-bed cabins fitted with 24 turnable beds Living Quarter with a
helideck.

Other main information is:

2. PURPOSE
2.1. General Considerations

Before a PTW can be issued for a task, it will often be necessary for the equipment or systems being
worked on to be isolated from sources of energy that might jeopardize the safety of working personnel.
The Isolation Certificate ensures that the correct isolations are applied before work starts, and that they
are not accidentally removed while the work is in progress.
Isolation is required before an energization can take place in order to separate the source of energy
downstream the system energized.

2.2. Scope

This procedure establishes the requirements for the electrical isolation used in conjunction with Lock
Out/Tag Out (LOTO).

These requirements are designed to ensure that the working environment of all personnel is made safe
whilst working on and/or around equipment and/or sources of energy.

This procedure is applicable during onshore commissioning phase at SHI yard.

This is a supporting document of SHI PTW procedure which has to be applied for all works on handed
over systems (RFCC signed off) within commissioning areas. However, if necessary for safety reasons,
can be applied also before RFCC issuance.

4.1. Definitions
Words appearing in capital letters shall have the meaning assigned to them in the CONTRACT Article 2.1
Definitions.
In particular: COMPANY means: TOTAL E&P NORGE AS

VENDOR means: The Company nominated in the Order issued by

WORKSITE(S) means: The SITE and/or all and any construction location(s), where

4.2. Process Definitions

Energy Source
Any source: electrical, mechanical, hydraulic, pneumatic, chemical, thermal, or other energy.

Energised
Connected to an energy source or containing residual or stored energy.

Energy isolating device


Is a Mechanical device that physically prevents transmission of release of energy e.g. CB, MCB, MCCB,
Fuse, Disconnecting Link, etc.

Equipment
Includes all equipment connected to a source of electricity.

Isolation
Is the process of physically separate one or more equipment from a potential source of electricity.

De-isolation
Is the reverse process of isolation by connecting back one or more equipment to a source of electricity.

Lock Out
Installation of a padlock on an energy isolating device, ensuring that the energy isolating device and the
equipment being controlled cannot be energised until the LockOut device is removed.

Padlocks shall be key operated and shall only be used for this procedure. Padlocks shall be utilised and
be unique and numbered. They are logged in the Electrical Isolation Register.
Padlocks used by Electrical Commissioning Team can have a master key dedicated per voltage level.

Tag Out
The placement of a TagOut device on an energy isolating device, in accordance with this procedure, to
indicate that the energy isolating device and that the equipment being controlled may not be operated
until the LockOut and TagOut being removed.

LOTO Tags
Tags are attached directly to the lock shank or by a tie-wrap. Each tag shall be identified with the
authorized person name, phone number and date of placement and relevant EIC number

Electrical LockOut and TagOut philosophy


The primary method of LOTO for energy isolation shall be both Padlock and TagOut. The Padlock
devices used in isolating the energy shall be distinctive and only used for this procedure. Padlocks shall
be key-operated and tagged to identify the authorised person who applied the padlock.

A Tagout system alone does not provide positive means of controlling stored energy.

Is used to control and monitor the isolation/de-isolation status in conjunction with the PTW system.

5. RESPONSIBILITIES AND AUTHORITY


The roles and responsibilities related to the electrical isolation methodology are identified on the following
paragraphs.

5.1. Individual Responsibilities


All individuals including Contractors and Subcontractors Employees working within all commissioning
areas/systems shall:
• Have a detailed working knowledge and a good general understanding of the SHI Permit To
Work Procedure
• Ensure they do not start work on any job requiring a PTW until one has been authorised and
issued. Permits will not be issued until the Authorized Person is satisfied of the individual’s
knowledge of the system
• Ensure that the conditions and precautions set out and specified in the permit issued for work,
which they will be involved with, are fully understood
• Ensure that all precautions and safety measures specified in the permits and instructions are
strictly adhered to
• If any circumstances or conditions change, seek advice from the Authorized Person and/or
Area Authority immediately
• If in doubt stop and ask for assistance
• Carry out all work in a professional manner
• Personnel involved in the managing of PTW System including Isolation process shall have
received necessary training

5.2. Authorized Person

The Authorised Person is the Commissioning Manager or his Deputy.

The Commissioning Manager or his Deputy shall

• Verify that this procedure is implemented


• Verify that people involved are trained and demonstrate a sufficient level of competency.
• Define the different Commissioning Areas.

• Assign a Commissioning leads to each Area and designate them as Area Authority
• Inform SHI about Area and Area Authority
• Coordinate the Permit-To-Work Submitted

• Evaluate and agree accordingly with the area authority the Competent Person proposed

The Authorised Person is the entity who authorises the “Permit” to the person in charge. He has overall
responsibility for the implementation of the PTW Procedure which involves any works within
commissioning areas/systems and shall review and approve permits affecting the commissioning
area/system before the work starts

He shall ensure that:


• All works requiring a PTW are identified;
• The PTW and associated certificates contain a clear description of the work to be done, its
location, start date and time, plus its duration;
• The safety precautions to be taken and any isolations required are clearly detailed on the
PTW and the associated certificates;
• PTW for work activities that may interfere or affect one another are cross referenced clearly
and effectively;
• All other works which would create a hazard, if undertaken at the same time, is risk assessed
and applicable controls be put in place to eliminate or mitigate the hazard.
• Permit Risk Assessments or Job Safety Analysis are performed, as deemed necessary;

5.3. Area Authority

The Commissioning Manager or his Deputy designates the Area Authority. The Area Authority is in
charge of one Commissioning Area. He is responsible for all activities ongoing, after issuance of Ready
For Commissioning Certificate, within his area (Construction or Commissioning activities).

The Area Authority is the Utility Commissioning Lead, the Process Commissioning Lead or the
Electrical Commissioning Lead

He shall, under the Authorised Person responsibility, verify and maintain the integrity of the electrical
isolation, during the full duration of the permit to work where LOTO is required

The Area Authority shall:


• Review and approve the isolation plans prepared by Electrical Competent Person
• Confirm that isolation plans are appropriate for the work being performed
• Check physically that isolation and tagging have been implemented correctly as per isolation
plans before work commences
• Validate and close the EIC
• Carry out site inspections before, during and after the work is performed.
• Confirm that equipment/system is safe to return to service before approving de-isolation.
• Verify that all permit and certificate forms have been correctly completed, signed and dated

5.4. Electrical Competent Person

An Electrical Competent Person is designated. The Competent Person cannot be a Performing Authority
or an Area Authority.

Isolations shall only be performed by competent person who has demonstrated competences in the
preparation of isolation plans and isolation of plant and equipment

The Electrical Competent Person has the technical knowledge and competence of electrical systems and
has been appointed by the Area Authority to identify and supervise the electrical isolations/de-isolations
done by the Performing Authority resources.

The Electrical Competent Person is responsible for:


• Preparation of isolation plans and the EIC according to isolation philosophy
• Verification of the safe implementation, by the Performing Authority, of the approved isolation
plan (by Area Authority).
• Verification of the tagging and locking of isolations, done by the Performing Authority, as
required by the isolation procedures
• Verification that equipment is safe to return to service before de-isolation
• Verification of the safe de-isolation of plant and equipment, by the Performing authority, with
locks/keys returned
• Keep the records of the positive isolation

5.5. Performing Authority

The Performing Authority is the person who is in charge of the worksite. The Performing Authority can be
either a Contractor or Subcontractor personnel (supervisor, engineer, discipline lead….)

All Performing Authorities shall have attended the PTW course and have been assessed as competent as
Performing Authority.

The Performing Authority shall:


• Ensure that the scope of work is clearly described on the PTW,
• Ensure that all hazards associated with the work are clearly described on the PTW,
• Have a clear understanding of the preparations, precautions and PPE required to carry out the
work safely,
• Be responsible for housekeeping at the worksite during the work activity, and ensure that the
worksite is left in a clean and safe condition once work is complete,
• Confirm that all worksite preparations are in place before work commences,
• Confirm that the correct PPE is available and that personnel understand how to use it correctly,
• Have a PTW signed by the Authorised Person prior the commencement of any work,
• Ensuring the Area Authority and/or the Competent Person are made aware of any changes in
work conditions or work content,
• Ensure that people executing the work are competent and trained,
• In the event of an emergency at or near the work site, ensure that all equipment used in the
work is safely shutdown,
• Carry out the isolation / de-isolation and tagging under the supervision of the Competent
Person.

6. ISOLATION PHILOSOPHY

prevent unauthorized operation during work activities.

6.1. General Electrical Isolations

Electrical isolation and de-isolation process enter in force after RFCC has been signed, before first
energization of the system/ subsystem. At this time all electrical equipment incomers and feeders will be
isolated, tagged out and locked by Commissioning Team as a consequence of sub-system RFCC
signature. For this purpose an Electrical Isolation Certificate (EIC) is not necessary, but all isolation will be
recorded in the Electrical Isolation Register (Attachment 1).

Each discipline/ subcontractor that requires isolation will prepare an isolation request by filling an EIC, and
after implementation, it will be signed and attached to their work permit (Attachment 2).

For changing the isolated state of one or more feeder, during commissioning activities, the requestor must
fill up and get approval of the MCC Switching Request (Attachment 3) in one of the position “Test”,
“Service” and “Isolated”. A condition for approval is that all EIC for the requested items are closed.

Before the isolation is removed for testing/ energization purpose (control or/ and main voltage) the entire
electricity path that are to be linked outside the system to non RFCC equipment have to be isolated;
sometimes cable disconnection is required and this will be recorded and properly identified as Temporary
Modification.
When a sub-system/ equipment is Ready for Energization the relevant isolation/ blocking point will be
removed (de-isolation) and moved downstream.

The disconnection shall take the form of an air gap or equally effective insulation which will ensure that
the point of disconnection does not fail electrically. Acceptable methods of electrical isolation are:
• Rack-out (disconnecting) the withdrawals units (CBs, ACBs, MCC feeders);
• Open CBs and separators (100kV GIS);
• Open MCBs, MCCBs, remove Fuses and separator links;
• Disconnect cables/ wires;
Where is provided, earth switch is to be closed, unless a test require to be opened.

6.2. Electrical Consideration

The electrical isolation has always to take in consideration the following measures:
a) Disconnecting completely
b) Securing against re-connection
c) Verifying that the installation is dead
Also, when is required has to be performed one or both procedures:
d) Earthing and short-circuiting
e) Protection against adjacent live parts
A special attention has to be paid on the equipment with redundant power sources (example: DC control
voltage supplied by two separate lines through diodes: the diodes cannot be considered electrical
isolating devices).
In case of wire/ cable disconnection, those has to be isolated by isolating tape or/ and isolating end cap
and tagged with warning message and information regarding identification of terminal block, cable/wire

number, date of disconnection, person in charge, phone number.

6.3. Control of Isolations


6.3.1. Isolation/De-isolation Sequence

Refer to Attachment 4.

6.3.2. Identification
The competent person must assess the work area to determine what equipment is being worked on,
and/or what nearby equipment may pose a hazard and needs to be isolated/ locked and tagged out of
service.

This includes reviewing drawings of the entire system to be de-energized/de-activated to determine what
must be isolated.

Physical inspection of the system must also be performed: to ensure the isolation points identified are
adequate; to verify isolation points; ensure drawings are accurate and ensure all isolation components are
in acceptable condition.

6.3.3. Tagging
The Electrical Competent Person designated by Area Authority shall tag each point of isolation. Tags shall
only be installed once the isolation is in place. The tag shall be filled out with information like name, phone
number, department organisation, date by using a permanent marker or pen and attached to the isolation
point directly or using a cable ties. When removing tags, cable ties shall be cut off, and not re-used.
Nobody has authority to operate or interfere with isolation while it has an isolation tag attached.
When de-isolating equipment, the tag should be removed from the isolation point before the isolation is
reinstated.

6.3.4. Locking
Isolations must remain secure through the duration of the work activity.
All Locks will be marked with an individual identification. Personal Safety Locks will be issued to each
person and it will be the responsibility of that person to either have that lock in their possession or
attached to a Group Isolation Board or a Multilock Device at all times while in the workplace.
Locks and keys will be made available from Area Authority for visitors and contractors and will be marked
accordingly with a Personal Safety Lock Identification Tag.

Padlocks shall be individually keyed and the keys shall be retained in a suitable lockout box. Keys to the
lockout box shall be held as a minimum by the Area Authority.

6.3.5. De-Isolation and EIC closure

Once work on a device is complete, the Performing Authority shall inform the Competent Person, and
then de-isolation can be performed and EIC closed.
If de-isolation is required, this can take 2 forms: in “test” position or in “service” position.

During commissioning, if de-isolation is not neccessary, the respective equipment will remain isolated until
a de-isolation will be requested through an MCC Switching Request.
