Subject: Orientation on the Preparation and Checking of School Forms

Date: June 9, 2022

1. After attending the division virtual orientation on the preparation and checking of school forms last

two days ago, this office have seen the need for relaying the information to teachers to compressively

be guided of the preparation. In this context, we are announcing to you the schedule of this

orientation which would happen on June 13, 2022 at 9:00 o’clock in the morning.
2. School Forms will also be checked by the Division Checking Committee thus, we should not be

mistaken in our entries.

3. Matters relating to transfer of school records, and enrolment policies will also be reviewed.
4. Finalization of school form entry will be done after the orientation.
5. Issuance of this memorandum is for your compliance and guidance.
