AFFIDAVIT OF PARENTAL ADVICE ON MARRIAGE

state:

2. That my/our child is intending to contract marriage with (PARTNER INTENDED TO

MARRY), in (PLACE OF MARRIAGE).

3. That I/we hereby advise my/our child to marry Ms./Mr. (PARTNER INTENDED TO

MARRY) legally, and to live together, observe mutual love, respect, and fidelity, and

support each other for as long as you live;
