Good morning everyone today we are going to discover what Module 5 has instore for us with
the topic BUILDING THE TEACHING PORTFOLIO RELATED TO THE IMPLEMENTATION OF THE
MODALITIES. But before we start I would like to congratulate everyone since we have made it
to module 5. After accomplishing the activities and outputs from module 1 to 4, we are now
ready to apply what we have learned from the previous modules. In this module we are going
to plan on how we could capture the proof or evidences of our teaching practices.
Slide 2: Objectives

Module 5 stands with 2 objectives the first one is to list down the evidence of LDM. And 2 nd is
to plan how to make sure that you will be able to relate these evidences to professional
practice based on the professional standards. The duration of this course is 4 hours.
Slide 3:

Let us now discuss its contents. LDM module 5 is comprised of 2 lessons. Lesson 1 covers the
TEACHING PORTFOLIO AND ITS CONTENTS it consists of 5 activities, while Lesson 2 which
covers the EVALUATION RUBRIC consists of 2 activities. So in Module 5 we are going to answer
5 activities in lesson 1 and 2 activities in lesson 2.

Let’s explore Lesson 1 (THE TEACHING PORFOLIO AND ITS CONTENTS).


Slide 5:

A portfolio is a compilation of our work course from module 1 to module 4, where it serves as
evidences of our learnings from LDM 2. A portfolio is an assembled evidence for the purpose
of 1. Evaluating the quality of our work, 2. To determine whether we have met the teaching
standards set for us, 3. It help us reflect on our career goals and progress as a teacher. Lastly
no. 4. A portfolio creates a lasting archives of work accomplishments. Portfolios can be in
Digital or in Black and White form.
Slide 6:

At this point let us identify now what should be there inside our Portfolio?
For module 1 we are expected to submit Lesson 1 with 3 activities, Lesson 2 with 2 activities,
and there is no output for this module.
For Module 2 we are expected to submit Lesson 1 with 4 activities, Lesson 2 with 1 activity and
1 Output which is the Unpacking of Sample MELC.
For module 3a we are expected to submit 3 lessons with 14 activities and 5 Outputs; These
outputs are the List of targeted Learner Intervention, the Learning task of Distance Learning,
the Assessment Methods in Distance Learning, the Weekly Home Learning Plan and lastly is
the Individual Learning Monitoring Plan.
For Module 3b we are expected to submit 3 Lessons with 8 activities and 2 Outputs where we
are tasked to provide 2 LR Rapid Assessments.
For module 4 we are expected to submit 2 Lessons with 5 activities and 1 Output which is the
Individual Development plan.
For this module, module 5 we are expected to submit 2 lessons with 7 activities and a Portfolio
as our Output.

Let us familiarize ourselves with the Lessons and activities of Module 5.


Lesson 1 Activity 1 is a checklist that aims to track whether we accomplished everything from
modules 1 to 4.

Lesson 1 activity 2 is composed of 3 questions. It focuses in Reflecting on Building your


portfolio.
Question no. 1 Our Portfolio serve as our evidence, a record of our accomplishment in black
and white, therefore we can track our progress of teaching practices with our portfolio.
Question no. 2 , These are; the Unpacking of MELCs from module 2. The Learning Task for
Distance Learning from module 3a. The Weekly Home Learning Plan from module 3a. The
Individual Learning Monitoring Plan from Module 3a. And the Individual Development Plan
from module 4. These are the evidences could capture our progress in our teaching practice.
Question no.3 Reflections were an integral part of our portfolio because it summarizes our
insights and experiences. It was like our very own time capsules were we kept important
details of our lives and soon be opened for us to recall the best in things that we
accomplished.
Slide 9:

For Lesson 1 activity 3 we are going to familiarize our self with the List of Suggested Evidence
Form, the said Form is composed of 3 columns , on the first columns is the ACTIVITIES (these
teaching stuff that we are expected to accomplish after completing the LDM 2 for teachers ),
on the 2nd column is the TIMELINE our timeline can be used to visualize time, durations that we
sets to for us to accomplish a certain task, on the 3rd column is the expected outputs or the
results of our activities from the first column.

Here are the Other types of evidences relevant to our Modalities. As an example we can Make
a list of targeted Learner Intervention from August to September and our expected output
would be a List of targeted learner intervention.

After familiarizing ourselves with the List of suggestion evidence form let us try answering the
follow-up the questions still from Lesson 1 activity 3.

Lesson 1 activity 4 is our reflections from activities 2 and 3, again activity 2 is all about tracking
our progress using our portfolio and activity 3 is all about familiarizing ourselves with the list of
suggested evidence form.

On Lesson 1 Activity 5 we are going to finalize our list of evidences but since the list is
suggested by the department of education it is valid and relevant, minimal alterations is
allowed to further localized the list.

Let us move on Lesson 2 which covers the EVALUATION RUBRIC

Here is the Criteria and indicators in Evaluating Portfolio. This RUBRIC is composed of 6 criteria
that will guide us in accomplishin our Portfolio.

On this Lesson we are going to study the Criteria and indicators found in the EVALUATION
RUBRIC. These RUBRIC will serve as our guide in completing our Portfolio. In order to achieve
an excellent grade under this criterion, THE OUTPUTS SHOULD BE CLEARLY AND ADEQUATELY
CAPTURED THE PROGRESS OF THE IMPLEMENTATION OF THE LDM, SHOWING INOVATIONS
THAT CONTRIBUTE TO ITS SMOOTH IMPLEMENTATION.
Slide 17:

Next is the QUALITY OF REFLECTION which weighs 25% in the rubric and in order to get the
excellent grade on this criterion your reflection describes in the LDM implementation, shows
in-depth analysis and synthesis and excellently relates it to the professional standards and
personal goal as a teacher.
Slide 18:

Next is the DEMONSTRATION OF PROFESSIONAL STANDARDS for 20% and in order to get the
excellent grade for the criterion Your outputs should clearly demonstrate 5 or more indicators
of relevant strands in professional standards. The 4th Criterion with 15% in the rubric is the
LANGUAGE AND OVERALL PRESENTATION OF THE OUTPUTS and to get an excellent grade your
ideas should be expressed in clear coherent and appropriately worded language with no
errors.
Slide 19:

The 5th and the last criteria shares both 5% in the rubric. THE ORGANIZATION OF THE
PORTFOLIO requires that the portfolio should be Logically Organized activities and outputs
were placed in a logical order. And lastly the TIMELINESS and in order to get an excellent grade
for this criterion OUTPUTS SHOULD BE SUBMITTED 3 DAYS AHEAD OF THE DEAD LINE.

After Studying the criteria and indicators found in the Evaluation Rubric we are now ready to
answer the question in Lesson 2 activity 1. What steps will you take to ensure that you will be
able to hit the criteria on the rubrics?
Slide 21:

And Finally the Lesson 2 activity 2 this part is for reflections where we are going to discuss and
share our thoughts on the Rubric. Present our steps to make our outputs and Portfolio
responsive to the evaluation criteria and indicators.
