3.0 Method Statement

1 Site preparation Instructions:

i. Work clearance form (WCF) must be filled up before start work
and inform dealer about the work. If site closed and do not have
dealer, WCF will be sign by PM maincon.
ii. JHA is available and present during work progress.
iii. Conduct toolbox meeting before start any work with all
workers.
iv. Fill up lifting Checklist and do skylift inspection.
2 Barricading the Instructions:

i. Securing the working area with hard barricade.

ii. Identify the working area and follow the practices below:

• Ensure the working area is barricaded accordingly. Working area

should be within a safe distance from vehicles and pedestrians.
There should never be any loitering or unauthorized personnel in
the unloading area.

• Ensure clear warning signals are displayed at all times.

Ensure working area is free from hazards that can compromise safety.

3 Position the sky lift Instructions:

i. Check ground condition – Upon setting up of the sky lift, ensure

ii. Ensure the skylift operator use suitable pads and cribbing –
operator and permit holder need to ensure that they are using the

3. Permit holder should perform sky lift inspections upon
commencing. Ensure the mechanical parts, oil, gas, fluid level and
other safety features is in good operating conditions.

4. Flagman should be present in order to ensure the smoothness of

the road traffic operation and to alert the road users.

4 Install the Golden Two workers will use skylift and marking the position of the Golden Arch.
Arch The cover of the Golden Arch is removed. The back panel is lift by skylift
and install on building.

Drill the holes by using cordless hammer drill. 12 holes drill for Golden Arch.
Wall plug size 3/8 is used to install the Golden Arch. Then tighten the wall
plug using rachet.

After finish tighten the wall plug, install the cover of the Golden Arch.

5 Housekeeping Remove all debris and clear the work area. Make sure barricade and
warning sign remove from the site.
