・COVID-19 vaccination is free of charge.

・What to bring:
Vaccination coupon ② ID ( Health insurance certificate, Driver’s license, etc.)
Completed Yoshin-hyo, Preliminary examination slip ④ Okusuri-techo, medication record
notebook ( If you have one.)

・Be sure to wear a mask when you go to the vaccination venue.

・You can cancel your reservation on the web up to three days before the vaccination
date. Cancelation after two days before the vaccination date is accepted by phone to the
Call Center. (℡：0120-567-970 9:00 a.m.～5:00 p.m.)

・If you enter your cell phone number on the reservation site, around two days before
your first dose, you will receive a text message to notify what to bring, etc. (Only those
who have chosen a group vaccination venue or a hospital venue will receive a message.)

Basically, you should make a reservation for the second dose of your vaccination on your
own. For the details, there will be instructions in the venue. (The procedure might differ
depending on the venue. Please check at the venue.)

・Web reservation system is only for those who have vaccination coupons issued by
Fuji City.

・ If you have vaccination coupons issued by another municipality, please call the
Call Center first.
