DIVISION MEMORANDUM
No. 325, s. 2022
1. This Office announces the acceptance of applications for Head Teacher positions
in Elementary and Junior High Schools in the Schools Division Office of Rizal. The
qualification standards are as follows:
A. ELEMENTARY LEVEL
2. All interested applicants shall submit the clear scanned copy of the following duly
authenticated documents through email at rizal.records@deped.gov.ph with the
subject: APPLICATION FOR ______________ (position applied), Attention: Division HRMO,
DepED Rizal:

all documents in portable
document format (pdf) only
a) Letter of Intent (addressed to Susan DL. Oribiana, LOI_(name of applicant)
Schools Division Superintendent. Kindly include the
position you are applying for.
b) Duly accomplished Personal Data Sheet (PDS Form PDS_(name of applicant)
212 Revised 2017) and Work Experience Sheet
c) Updated Service Record SR_(name of applicant)
d) Latest Appointment Appointment_(name of applicant)
e) Special / Office Order of Assignment, if any SO_(name of applicant)
f) Transcript of Records in bachelor’s degree and / or TORBachelor_(name of applicant)
Certification in Professional Education and Diploma TORCPE_ name of applicant)
DIPLOMABachelor_(name of applicant)
g) Latest Transcript of Records and Diploma (Masteral/ TOR_(name of applicant)
Doctoral Degree) DIPLOMA_(name of applicant)
h) Certificate of Complete Academic Requirements issued CAR-MD_(name of applicant) OR
by the University or College Registrar if not graduated CAR-DD_(name of applicant)

3. If your documents are stored in your google drive, access shall be given to
rizal.hrmo@deped.gov.ph upon submission of documents for fast conduct of initial
evaluation. Applicants who will not be able to give access to the abovementioned email
address will be disqualified automatically. Applicants who have already submitted their
application documents are required to submit their complete scanned documents
through the abovementioned email address.
4. All applicants must accomplish the google form upon submission of documents,

5. Documents will be evaluated using the DepEd Order No. 42, s. 2007, The Revised
Guidelines on Selection, Promotion and Designation of School Heads.
6. Original copy of documents must be presented during the face to face conduct of
evaluation of documents, thus, only those documents will be given corresponding
points.
10. Schools and Division Sub-Office shall post this Memorandum on their
respective website, FB page, other media pages, and bulletin boards. The
Administrative Officer II in the central school shall ensure the posting of this
Memorandum in their respective Municipal Public Employment Service Office.
Proof of posting such as pictures and screenshots of website / FB page / other
media pages shall be sent at hrmpsb.secretariat@deped.gov.ph.
