March 16, 2022 ID’s and booklets of our clients
from barangay Sinaja
 Validated 1 walk in clients, they
are the waiting list from barangay
Concepcion.
March 17, 2022  Helped JO-San Shyne Kristiansen;
on the replacements of 2 ID’s of
senior citizens from barangay
Poblacion.
 Attended 1 walk in client from
 Office Works
barangay Sinaja inquiring for; UCT
Social Pension schedule payout
and explained were still waiting
from regional office for the update
of UCT social pension.
March 21, 2022  Attended 7 walk in clients inquired
regarding senior citizens
requirements for membership and
to acquired ID's and Booklet. They
 Office Works/Administrative accomplished registration forms.
Tasks  Assisted 3 walk in clients from
barangay Rombang, inquired
about SOCPEN requirements to
apply as waitlisted. I gave them
the application form and they
accomplished the form.
March 22, 2022  Assisted SDO for the releasing of
first quarter 2022 Social Pension
stipend. Elders are very happy
and thankful in receiving their
 Assisted at Municipality of San stipend. Due to lack of teller we
Remigio Pay-out opted to help even though we do
not have a special order, since
Municipality of San Remigio
generally have a far-flung
barangays due to this they need
to have a tellers to catered them
to be able to finish and go home
early because they need to walk
3-4 hours until they reach their

own barangays. Municipality of
Belison is nearby Municipality of
San Remigio and it is not a
hindrance to me if I augmented
the only risk is my safety. And I
think it is better to have an
enough number of tellers to the
municipality considering that
they belong to the
Geographically Isolated and
Disadvantaged Areas (GIDA).We
served barangay Bawang (17)
and Sinundolan (164) in the
morning and in the afternoon we
went to Aningalan Gym and
served Bulan Bulan (17), Osorio II
(61), Osorio I (38), Agricula (32),
Nagbangi II (27), Nagbangi I (52),
La Union (70), Tubudan (45),
Aningalan (36)

March 23, 2022  Assisted SDO for the releasing of

and thankful in receiving their

March 24, 2022  Augmented at Municipality of  Assisted SDO for the releasing of
Anini-y Pay-out first quarter 2022 Social Pension
stipend. Elders are very happy
and thankful in receiving their
stipend.

 I served barangay Nasuli C with
87 beneficiaries
 And in the afternoon, together
with SWA Patrick Bernesto we
went to Lisub A with 72
beneficiaries.
March 25, 2022  Downloaded new generated list of
senior citizens
 Crossed-match validated list
versus generated list.
 Assisted 2 walk in clients from
barangay Poblacion; inquired
 Office Works about KABALAKA Program
requirements. I gave them the
application form and they
accomplished the form.
Attended 3 walk in clients inquired
with regards to the requirements of
cash gift for senior citizens.
March 28, 2022  Updated the deceased Social
Pension beneficiaries
 I am saddened to know as I
updated the list of deceased the
newly replaced beneficiary for
the1st quarter passed away.
 Updated the list of the possible
names of beneficiaries for the 83
 Office Works
slots additional slots of
beneficiaries.
 Printed the list of deceased
beneficiaries
 Updated the masterlist in
preparation for 2nd quarter
distribution of stipend.

March 29, 2022  Made the response/feedback

report during the settlement of
centenarian beneficiary
Wencislao dela Cruz. And
submitted to Centenarian Focal
Doyien Castillo and cc the file to
PDO I Lovelyn Ocbian.
 As a SWA, I would like to request
 Office Works(Feedback Report, a copy of the guideline of
Encoding) centenarian, so that if there will
be the same case in the future
like what happened to Mr.
Wencislao dela Cruz, I have the
knowledge, basis and guide to
take action regarding
centenarian program.
 Encoded 14 accomplished SPBUF
forms during my field validation.

 Validated waiting list 1 walk in
clients from barangay Mojon.
 As of now Municipality of Belison
has 905 social pension
beneficiaries and 400 plus and
counting waiting lists. During
pandemic, Belison requested to
have additional beneficiaries
around 379 to serve the indigent
senior citizens in municipality.
 And now, Belison have 83
additional and still hoping to have
some additional in the future.
 Tackled the new updates and
additional beneficiaries per
municipalities; strategies and
initiative to improve social
pension program and distribution
of stipend subsequent to the
health protocols by the DOH even
though we have a scarce tellers;
delivered smoother and faster
pay out to the elders and not let
them wait since mostly they lived
in a far-flung barangays in
Antique.
