 Listed the finding (incorrect/inconsistent signature from barangay Poblacion)
 Counter-checked the spelling of the name in ID versus in cleanlist & payroll
 Validated un-served walk in client from barangay Mojon
February 3, 2022  Sort the photocopies and authorizations per barangay.
 Catered the inquiries of the walk in clients regarding the schedule of SOCPEN stipend
distribution.
 Informed Barangay Concepcion President regarding the unsubmitted ID of Delia Nolasco
and for replacement ID of Emelie Cantong
 Validated un-served walk in client from barangay Concepcion
 Listed the finding (incorrect/inconsistent signature from barangay Poblacion)
 Counter-checked the spelling of the name in ID versus in cleanlist & payroll
February 4, 2022  Catered authorized representative of Graciano Tanaman (Deceased), gave him the
Warranty and Release from Liability and inform him to attached original/certified death
certificate.
 Sort the photocopies and authorizations per barangay.
 Listed the finding (incorrect/inconsistent signature from barangay Poblacion)
 Counter-checked the spelling of the name in ID versus in cleanlist & payroll
February 7, 2022  Catered authorized representative of Semiona Tanaman (Deceased), gave him the
Warranty and Release from Liability and inform him to attached original/certified death
certificate.
 Catered the inquiries of the walk in clients regarding the schedule of SOCPEN stipend
distribution.
 Mrs. Fe Rapadas is inquiring when she will become social pension beneficiary

February 8, 2022  Sort the photocopies and authorizations per purok of barangay Poblacion
 Catered the inquiries of the walk in clients regarding the schedule of SOCPEN stipend
distribution.
 Validated un-served walk in client from barangay poblacion and sinaja.
February 9, 2022  Attended monthly meeting of PFSCAP.
 We tackled the following;
-Schedule of payout (March 1, 2022), per barangay
-Completeness, findings & compliance of submitted ID’s and authorization for checking
-Foods and transportations of the tellers
-Informed the coordinator in barangay Poblacion with regards to their new replacements
 Catered 3 the inquiries of the walk in clients regarding the schedule of SOCPEN stipend
distribution.
 ML Pantawid Clayton, inquired regarding updates of Senior Citizen Centenarian
beneficiary Wencislao Dela Cruz, I informed him that Mr. dela Cruz is scheduled for 1st
quarter payout.
 Checked the photocopies of ID’s from barangay Poblacion Purok 7
 February 10, 2022  Checked the authorization letter of Generoso Agdalipe (out of town) with attached
plastic of LBC (proof that the documents is valid and not forged)
 Validated un-served walk in client from barangay Ipil
February 14, 2022  Informed the concerned senior citizens those who have findings and compliance as per
pre-checking of their submitted documents for 1st quarter payout.
 Catered the walk in clients; submitted their photocopies of ID
 Catered the inquiries of the walk in clients regarding the schedule of SOCPEN stipend
distribution.
 Validated un-served walk in client from barangay Borocboroc
February 15, 2022  Went to SWAD office, helped PDO I Lovelyn Ocbian to sort the money per barangay for
Municipality of Laua-an and Patnongon, in preparation for the distribution of social
pension stipend 1st quarter 2022.
