One of the easiest statistical instruments to formulate is a survey form. Surveys are
very helpful tool and useful in research among different fields. It is low cost, extensive,
flexible, and dependable. You will create your own today! Follow the steps below to
plan your survey.
STEP 1. Choose your population. (Encircle it)

STEP 3. Determine the thing/topic you are most interested to know. Choose from the

STEP 4. Based on your answer in step 3, write a short description for your survey.

present in your survey form. Decide whether your questions will be multiple-choice or
