1. Introduction
This method statement establishes how the Contractor carries out the Civil Works
for the construction of the gate house according to the scope of the Project, the
requirements / procedures according to the tentative schedule. In order to
complete work activities safely, the Contractor will carry out the work in
accordance with the relevant codes and standards and local regulations for Andres
LNG Terminal Expansion Project which is located in the facilities of AES Andres,
Punta Caucedo, Boca Chica, Dominican Republic.

1.1 Purpose
The purpose of this Statement of Method is to outline our approach regarding the
soil, formwork, reinforcement and concrete works to be executed for Gate House
within the proposed duration of the phases, under the highest quality standards of
materials. , Health, security and environment.
The purpose of this method statement is to establish the requirements;
- Provide a clear description and step-by-step guide to get the job done.
- Ensure that the quality requirements for the work that are specified and
implemented
- Detail the specific HSE requirements that must be considered for the completion
of the work.
1.2 Definitions
Related to the Project are as shown below.

2. Scope of Work
The work consists of the construction of the Gate House in AES Andrés, the works
to be carried out cleaning the areas of rock excavation, fill, line concrete, formwork
and concrete, always implementing our HSE plan and complying with the proposed
schedule

2.1 Contactor

2.3 Responsibility
Project Manager:
You must have all the activities coordinated with the quality, security and
administration team, and coordinate that the activities to be executed are in
accordance with the Method statement and with the security procedures, in the
same way provide the resources of equipment, approved labor plans and others
for all activities, participate in work meetings with the client for future jobs and
other needs. Verify the work schedule and compliance with its deadlines

HSE Manager:
 Socialize the relevant safety procedures for the job.
 Verify that the technical procedures implemented are carried out correctly and
safely throughout the development of the activity.
 Verify the use of PPE in the activity procedures.
 Conduct training before activities (HSE induction, among others).
 Carry out risk analysis in the activity to be carried out.

 Ensure that each activity has its APT duly formulated and signed.
Site Engineer:
This is a technical representative who is in charge of the day-day execution of the
work area, complying with the project regulations and must ensure compliance
with safety
Quality Manager:
Responsible for the elaboration of the procedures and method statement, for the
execution of the work to be carried out.
Supervise the correct execution of the work activities, as established in this
procedure.
Coordinate the human resources and equipment placed at their disposal, for the
execution of the works.
Approval of the materials and machinery to be used.
Quality supervisor:
Supervise the correct execution of the activities to be carried out, as established in
this procedure.
Stop the procedure in the event in which there is any inconsistency with the

Elaboration of ATS before the actity and depending on its scope

Personnel:
Generally, vary depending on the location in which they work and their level of
expertise. They usually work in different areas, as helpers or assistant of a specific
task, where they help to lift scaffolding, clear areas and help specialized workers,
such as painters and electricians

3. Politics
1) A surveying team will survey the area for excavation work
2) The personnel involved in these activities must wear the required personal
protective equipment, according to the work to be carried out.
3) The procedure must be documented before, during and after the application of
the treatment.
4) Each activity need to be verified, tested and released prior subsequent activity
5) The materials to be used must have their quality certificate and / or tests (as
applicable) in compliance with the specifications applicable to the project.
6) Temporary Facilities and Services, Mobilization and Demobilization

With the delimitation of the areas destined for these purposes, the Servinca
subcontractor proceeds with the necessary coordination (Operation and Security)
for the execution of the activities of mobilization of equipment and machinery,
temporary offices, parking lots, warehouses, stock of materials. , mechanical
workshops and temporary services

This must be done with those responsible for the project, as well as its operation
and safety departments, who will provide the access and control facilities
according to the regulations. The Contractor will guarantee compliance of the
statement and regulations during the entire mobilization and installation process,
execution and demobilization

4. WORK PROCEDURE (execution process order)


4.1 Site Survey
The surveying brigades will enter once they have the corresponding accreditation
and authorization to formally start the lifting and subsequent marking works

according to the indication of the engineer in charge and compliance with the
established work schedule. It is important to point out that before starting the gate
house foundation works, additional control points will be carried out specifically on
the base. For greater precision, the equipment to be used must be calibrated in a
certified company in the Dominican Republic.

These works will include the survey of all the areas to be intervened, whether they
are expansion or rehabilitation works of existing structures, establishment of
control points and verification of polygonal closure, the data captured in the
different areas must be stored and named in folders of different files to control and
facilitate their execution in the order described in the project schedule.

Area to excavate will be delimited with stack and flags. Total width should include
the foundation width plus space to put and secure formwork. Excavation depth to
be verify every two cut layers to assure proper final elevation (design elevation)

4.2 Clearing & Grubbing

These activities consist of cleaning, cutting, extraction and elimination of all trees,
stumps, roots, etc., in such a way that the foundation areas marked according to
the plans of new works, these works be totally free of organic matter and other
inadequate ones that could contaminate fillings or embankment platforms for new
structures and pavements.
The disposal of trees, stumps and roots must be carried out of the work area in
dump trucks duly equipped with protective mantles to prevent the fall of objects
throughout the journey to the final disposal of these materials.

4.3 Excavations
Before starting the excavations, servinca requests a plan of the area that indicates
the existence of pipes or electrical lines.
These activities consist of digging with heavy equipment. The total width to be
excavated for the foundations of the entry house will be marked with wooden
stakes, quicklime, and paint. A backhoe will be used to remove the excavated
material. The cut material will be loaded and transported within the project by
trucks. For Concrete Cleaning Works, the works will be carried out according to
the plans, the excavated levels will be rectified with the surveying equipment to

avoid over excavations and have the appropriate dimensions for foundations and
pedestals.
Access to the excavation should be constructed outside the useful area of the
excavation to avoid interference with structural elements.
To monitor the stability and drainage conditions of the trench, if necessary,
servinca will use formwork and wooden props as a shield for the stability of the
excavation of the wall. In cases where we need dehydration within the foundation
trench, servinca has flexible pipes and a submersible pump to proceed to drain the
water.
For signaling and barrier, Servinca will install 1.20m high wooden fences on both
internal and external sides along the excavation with its label, the barrier must be
installed not less than 0.70 m in reference to the edge of the excavation.

4.4 Lean concrete

It is used to provide smooth surfaces for the placement of structural


reinforcements mainly in foundations, at the same time it prevents the concrete
base of a structure from touching the ground, thus avoiding contamination or loss
of water during the setting of structural concrete that would be placed later.

This is important, as moisture and various chemicals in the soil can damage
concrete. For example, sulfate causes concrete to deteriorate causing it to soften
and break down you don't want to risk damaging the fundamental pillars of your
structure.

The concrete will be placed and leveled according to the established thickness of
50 mm with a tolerance as indicated in the document ELT-01-CQ-PC-0104 ITP for
Concrete Works for Concrete Works of ± 10 mm. if it is necessary to place a height
higher than the concrete line, this must be authorized by the client (Posco)

4.5 Rebar

The reinforcement bars will be received at the Company's base camp, where they
will be cut and bent, following the project guidelines. When finished, the rebar will
be shipped / transported to the project site campsite. Forklift crane / telehandler
will be used to unload platform trucks with bars.

workshop, steel that is exposed and dirty with concrete residue should be cleaned
with metal toothbrushes.

These pieces from bars final length and bending specifications will be in
accordance with the ACI 318-18 and approved by the Civil Manager Engineer of
POSCO E&C, alongside with the splices at due time. After that, steel fixer team will
cut and bend the steel rods with specialized machines, finally they will classify
every piece of steel and bundle for prior sending to Construction site.
When the steel is on site, dimensions, hook angles and the general condition of
the bars must be verified to guarantee compliance with the project specifications,
while the verification of quantities will be done.
Concrete shims must be used to ensure the specified clearance between the
surface and the bars. The concrete shims (spacers) must be constructed with the
same or greater resistance to the compression of the base, this to prevent them
from ripping

The lap splice of the longitudinal bar shall be made in accordance with the design
drawing.

Slab work
For the placement of steel on elevated surfaces, movements of loads must be
carried out, the personnel will be trained before the activity.

For works at height such as roofing, the assembly process will be the same with
the exception that the works will be carried out in height where suitable access
ladders will be installed, protection lines with wood or lifelines will be placed as
required by the client (posco E&C) must give the go-ahead Train the personnel
involved

It is worth mentioning that for the prior placement of the formwork for the works,
beam bottom levels and slab bottom level will be established, before assembly,
these will be reinforced with struts under the formwork wood, these struts will be
placed on a firm material on the ground to avoid deformation of the elements
(structure) the props will be steel or wooden bars. The process of this work is
described in the Formwork section.

Placement will be # 3 @ 200 in all directions, for main steel (Short span), and for
sub rebar (long span), The details of the Beam (RG1) will be: Top bar 2- # 4, Bott
bar 2 - # 4, stirrup 2- # 3 @ 50, the reinforcement of the beams will begin in the
two places the steel bott will be placed first, then the striip to complete the steel top
and then the lateral wooden caps that form the formwork will be placed.

4.6 Formwork

After rebar works has been cleared by supervision, formwork will begin in
accordance with calculation and tolerances in section 5.3 of ACI 347 and ACI 117,
reflected on ELT-01-CQ-PC-0104 ITP for Concrete Work, will be of -20mm/+25mm
for cross-sectional dimensions, ±10mm on deviation from elevation on top of
pedestal ±0.2% on formed surface over distance of 10ft and +25mm on formed
surface irregularities.
The forms will be made of wood to guarantee the shape of the circumference.
Formwork is required to delimit the shape of the concrete structures according to
the project drawings. It is very important to ensure that the formwork is strong
enough to withstand the pressure and vibration of the concrete pour, as well as to
keep the element dimensions within specified tolerances, as indicated on
approved construction drawings.

Prior to the formwork placement activities, topography will make the lines and
indicate the corresponding levels of the formwork according to what is indicated in
the engineering guidelines of the project.

Formwork surfaces must be clean, free of trash, mortar, and other material waste;
and have a uniform texture. Before reusing the formwork, they must be cleaned
and oiled with unmolding agent named form slip (find its datasheet on
attachments).

Form surfaces should be properly cleaned of "flaking" and debris. One way to
improve its condition is by cleaning it, oiling it and leaving it exposed to the sun for
a day; rubbing with liquid paraffin the affected areas or applying a thin layer of
lacquer.

For work at height such as slabs, the assembly process will be the same with the
exception that the works will be carried out at a height where suitable access
ladders will be installed, protection lines will be placed, with wood or lifelines as
necessary. Train the personnel involved. And the client (posco E&C) must give the
go-ahead, all the personnel involved will be previously trained.

All wood resulting from formwork work must be clean of any material that may
cause damage. (Nail, etc.)

In the assembly of the formwork, the following should be sought:

-Strength: To safely support the weight of concrete lateral pressure and


construction loads.

-Stiffness: To ensure that the alignment sections of the finished concrete do not
show any variation.

-Stability: To prevent the shoring from slipping, it must be verified that the
formwork is braced and secured in all the necessary points.

After installation is complete site supervisor must verify the following item:

-Water tightness: The joints must be hermetic so that the cement paste does not
leak.

-Support of upright feet: When the formwork is supported by struts or upright feet,
it is essential to verify that the supports or bases are fixed and stable.

-Secure and fix mold and reinforcement elements

-Verify shape, measurements, levels, stability and direction. Direction is especially


important because of the ring-shaped foundation.

All exposed edges need to be chamfered.

When inspector approves it, release agent will be applied on the contact surfaces
of the formwork to prevent it from sticking to the concrete and thus facilitate its
removal.

When applying these products, precautions should be taken to avoid damaging


the reinforcing steel or construction joints.

Unmolding will be done once the curing time indicated in the project specifications
has passed. The minimum time to remove the formwork will depend on the
strength of the concrete and the method based on the use of tests on concrete
samples, for schedule timing while maintain quality we recommend to do it after 24

hours of concrete placement.

Is important to remarks that specific structural design of formworks will be


submitted with clear section and dimensions of braces.

The props that support our house will be made of steel, these will be certified
props, as well as the access to the slab for the works, these will be scaffolding with
their certification.

To work at height, we will install a wooden sill, lifelines and the worker must be
properly trained and with their safety harness
4.7 Concrete

While concrete trucks are coming to site, prior to the beginning of the concrete
pouring, water must be sprayed on both the formwork and the steel in order to
prevent the accumulated heat in these elements from causing the concrete to lose
water when it comes into contact with them.

- To avoid concrete falling from more than 1.00 m, a Concrete Pump will be
needed alongside sufficient hose to guarantee it.
- Verification of the area to be placed, which must be free of residues or elements
not typical of the emptying.
- Once Mixer is on Site, we will verify the Slump as a method of measure the
workability of the mix (Contractor provide the require tools).
- Temperature measurement
- Taking samples for the preparation of cylindrical specimens (provided by
contractor, is not mandatory that Servinca take samples knowing that contractor
is responsible for given concrete)
- Avoid falling concrete from heights greater than 1.00 m.
- Verify that the concrete reaches all the points of the structure
- Provide a vibration that allows a correct compaction of the concrete
- Verify that the structure has the required levels
- Verify that the joints are according to the requirements
- While pouring the concrete, process will be done by horizontal layers not more
than 60cm of height and each layer must to follow the vibration process.
- Vibration process will be done with a proper equipment and a specialized brigade
that will come to that specific task, while the physical installation for pouring

concrete will be on metal scaffolding supported on steel externally and steel rod
internally (for foundation work, on pedestal will be supported in steel) to
guarantee proper height to the brigade to work.

- Concrete Curing procedure to prevent the increase in temperature from causing


cracks

- Conscious of the importance of proper curing as critical to the strength and


durability of concrete in foundation and pedestal.

- When a slab-on-grade foundation is curing, we will provide at least 24-48hrs time


for hydration to take place with proper chemical, after that we will keep curing
with water three times per day until 7th day. Reducing from two times to one time
until 28th day of placement. We are aware that Hot, dry conditions make water
evaporate more quickly from the slab surface, which can cause concrete to
harden more quickly, ultimately leading to a weaker material that's at greater risk
of curling and cracking. The surface of the slab will be kept moist to prevent
water from evaporating too quickly. We knows that inadequate hydration reduces
the slab foundation's strength, durability, impermeability, resistance to abrasion.

- In accordance with concrete plant, we will verify if the concrete strength at 24hrs
is enough to allow us to unmold a day after pouring, with appropriate approval
from Contractor.

Case of concreting in columns


For work on columns, holes of 30x30 cm will be made at a height of 1.5 meters, to
be able to place concrete and vibrate, this will be done in case the hose and the
vibrator cannot be inserted through the upper part of the columns

Desired concrete joints

If desired joints are necessary, a specific point will be taken to make a provisional
closure with wood, this closure will be provisional until the setting of the concrete
to make a treatment of the joint (Green cut), this for horizontal pour elements, the
waiting steel will be cleaned and free of any impurity

In the case of columns, the concrete is treated waiting, if necessary, a product will
be applied that is cut down by the client.

4.8 Backfill

These activities involve spreading, leveling and compaction of the material with
motor grader and roller compactor or manual leveling and compaction. The fill
materials will be produced off-site and transported to the project by trucks. These
materials must be tested prior to arrival on site to ensure compliance with project
standards.

This material will be placed on next to the foundation of Gate house, after the
proper curing process. The depth of the layers should not exceed 40 cm for the
roller compactor areas and 20 cm for the manual compaction areas.

The final phase of acceptance of this process is conditioned to the verification of


leveling according to the plans and designs, the control of the thicknesses and the
levels of the real surface will be inspected and verified by the owner or his
representative for the final acceptance of this phase each layer should have a site
density test per gamma density meter mark. Consulting the ITP ELT-01-CQ-PC-
0104 for concrete works for more details, knowing that the deviation of the
elevation of the upper surface as ACI117 will be ± 20 mm.

4.9 Banking
These activities involve spreading, leveling and compacting the material with a
motor grader and a roller compactor (Two Ton Roller Walker). Backfill materials
will be produced from two sources: mine pit work and site milling, these materials
must be tested prior to arrival on site to ensure compliance with project standards.

This material will be placed on the sides of the base of the Gate House, after the
proper curing process of the base. The depth of the layers should not exceed 40
cm for the roller compactor areas and 20 cm for the manual compaction areas.

The final phase of acceptance of this process is conditioned to the verification of


leveling according to the plans and designs, the control of the thicknesses and the
levels of the real surface will be inspected and verified by the client or his
representative for the final acceptance of this phase. Each layer should have a site
density test using a nuclear density meter. Each layer should have a site density
test per gamma density meter mark. Consult the ELT-01-CQ-PC-0104 ITP for
concrete works for more details, knowing that the deviation of the elevation of the
upper surface as ACI117 will be ± 20 mm.

4.10 Sequence of works observation.

Servinca will be in close coordination with the work to be carried out, to comply
with the proposed schedule, the excavation and the placement of steel according
to the plans, the concrete work and everything related.

5. Work place
During the execution of the activities, all loose material, debris or trash resulting
from the activity on the surface will be removed and disposed of in a designated
safe area outside the perimeter of the project (or classified trash cans) like pieces of
bars or wood, blocks.

The areas will be duly signposted to avoid incidents


The works carried out at height, precautions will be taken not to store objects and
properly marked areas.

6. HSE Plan
6.1 All the operational personnel involved in the activity must comply with applicable
requirements for health, safety and sanitary provisions controlling their operations
and those of the subcontractors and all suppliers, to assure the least
inconvenience to the project with a safety and healthy working environment.

6.2 All operational personnel involved in the activity must participate at the beginning
of their work in the Start of the Day Meeting (SDM) with a topic of Safety, Health or
Environment focused on the activities to be carried out during the shift.

6.3 It is the obligation of the construction leaders in conjunction with the HSE staffs to
make known to the personnel involved in the activity the Hazard Identification and
Risk Assessment (HIRA) related to their activity.

6.4 Before starting the activity, the Area Supervisor will show the personnel the
evacuation route, meeting point, identified audible alarms and colors emitted from
the point where the work will be carried out, this is if there is an emergency,
according to what is established in the Emergency Response Plan (ERP) of the

Project.

6.5 The Area Supervisor must verify the condition of the workers' PPE. In addition,
when applicable, use the specific PPE according to the project implementation
area.

6.6 You must verify that you have the Work Permit, Work Safety Analysis (WSA) and
checklist released and authorized by the person in charge of HSE.

6.7 The HSE Supervisor must be present at the work area where there is a risk
detected or indicated in the Critical procedures.

6.8 Construction personnel, including equipment operators, must be trained and


qualified in operating procedures, as well as in safety, both critical and specific to
the project, according to what is indicated in the project's Safety Plan.

6.9 Equipment operators must be qualified and trained in the use and operation of the
equipment.

6.10 All maneuvering personnel must be qualified and trained in the maneuvering and
warning signals necessary for the development of the activity.

6.11 The necessary and adequate required signs must be placed to delimit the work
area, in order to indicate the dangers and risks to which the personnel is liable in
their work area, to prevent any type of incident.

6.12 An inspection of the equipment and machinery must be carried out by the operator
and the maintenance department in conjunction with the HSE to determine that its
functioning and operation are adequate, prior to the start of activities.
6.13 The operating manual of the equipment used in the activity must be in Spanish (or
English, as the case may be) and must be known to the personnel involved in the
activity.

6.14 In the event of an emergency, the leaders of each area should inform the HSE
personnel in their area. The person in charge of HSE will immediately inform or
notify the corresponding personnel of what happened.

6.15 The staff must know and act in accordance with the established Emergency
Response Plan.

6.16 The construction team in coordination with the HSE supervisor must identify and
evaluate its environmental aspects of the activities to be developed, in accordance
with the provisions of the procedure for identification and evaluation of
environmental aspects.

6.17 The management and disposal of solid urban waste, special management waste
and hazardous waste will be carried out in accordance with the provisions of the
comprehensive waste management plan.

6.18 In the event of a spill, the staff must act in accordance with the Environmental
Surveillance Plan.

6.19 All personnel involved in the activity must have a talk about the Environmental
Aspects that are present in their work area, the established operational controls,
as well as the actions to be taken in case of any impact.

6.20 Maintenance personnel must have a Spill Kit when performing any maintenance or
repair of any equipment.

6.21 The ring excavation area should be delimitated by mobile railing.

6.22 Proper ladder should be constructed prior rebar works start
