Job Safety Analysis Management Procedure


1 Purpose

To control risk and make sure people, equipment and facility in safely condition.

2 Scope

This regulation applies to company, wholly-owned subsidiary and non-wholly-owned holding

subsidiary.

3 Definition

3.1 Job Safety Analysis (JSA)

It is a method that identify hazard and evaluate risk prior to performing a job, and properly

make and carry out control measure in accordance with risk assessment, to maximize risk

eliminating and reducing.

4 Responsibility and authority

4.1 Units

Manage and maintain JSA according to this procedure.

4.2 QHSE department

Provide consulting and supporting and audit.

4.3 Operation leader

a) Assemble JSA team member to complete JSA.

b) Organize the JSA in accordance with this procedure.

c) Make and carry out the risk control measures.

Participate in the process of JSA, identification, recognition of job risk, and carry out control

measures.

5 Management content

5.1 JSA apply to the following operation activities and situation:

a) Non-routine job without procedure (no guideline, work plan and operation procedure)

controlled activity;

b) A job which has an unacceptable or changed condition, and be deviation from standards

and procedure.

c) A job which need to make work plan and approval.

5.2 The steps of JSA

The JSA contains 5 steps as below:

5.2.1 Step 1: Confirm subject

After confirm JSA for job, fill job description in “job description”(refer to appendix 1), e.g.

lifting, repair an equipment, hot work in an flammable and explosive places.

5.2.2 Step 2: Break down the task as several steps in sequence

Break task down as several critical steps and use one verb word to describe each step when

carry out JSA.

5.2.3 Step 3: Identify hazards from each step

Identify the involved person, machine and environmental to each step which include tools and

objects; to survey work condition; to identify the risk and probable consequence from each step.

Then fill the potential hazard type and consequence in "probably consequence" section, e.g. serious

injury due to falling objects, hit ankle by object dropped from high. The below items need to be

checked and confirmed (include but not limited):

a) Did any health, safety and environmental issues and incident occur before?

b) Any new equipment will be used in this job?

c) Check the work condition, space, light, air, entry and exit.

d) Check the critical step of this job;

e) Does the persons who have enough knowledge and skills to perform?

f) Does the job need a PTW? The type of PTW will be?

g) Is there any Simultaneous Operations will impact task?

5.2.4 Step 4: Make control measures.

Make sure control measure of eliminated or reduced identified hazards (include emergency and

first aid measure), refer to appendix 2 “The hierarchy of risk control measure flow chart”.

5.2.5 Step 5: communicate and carry out control measures.

Once confirm control measure, these measures should be understood the risk, control measure

and personal duties in the task by related persons, before commence.

5.3 Relevant requirement

5.3.1 JSA should be re-conducted if any new hazard has been recognized or a near miss and incident

have occurred during this job.

5.3.2 Persons involved shall participate in JSA process actively. When job task finished, operators

should draw experiences and develop operation procedure for this job according to the process of

doing this job. This type of job can be carried out as the procedure instead of JSA in next time.

5.3.3 For those jobs with routine procedure, it can be referred to this procedure as well. In the tool

box meeting, carry out an oral JSA mainly by operators as a way to train stuff in knowledge, skills

and make people involved in safety activity.

5.3.4 JSA is a simple but useful risk management tool. Company requires all onsite production,

supervision and safety staff to be skillful at JSA. All level of direct leaders and safety staff should

participate in or sit on the JSA process, and provide supervision and guideline to keep improve in

the quality of JSA. Each unit should set JSA as a critical content of audit during auditing, organize

JSA special audit when necessary.
