1 Purpose

Identify and control office risks effectively, provide necessary QHSE guideline to office staff.

2 Scope

This regulation covers all corporation and its wholly owned subsidiaries, those non-holly

owned holding companies can make implementation with reference.

3 Responsibilities and accountabilities

3.1 QHSE Management Department

a) Compile, revise and explain this procedure, instruct and supervise the implementation of

this procedure.

b) Supervise and instruct office QHSE management.

3.2 Supply chain department

a) Manage office QHSE.

b) Check and record office QHSE management implementation periodically, and do good

record-keeping.

c) Command and direct emergency evacuation and rescue, implement office QHSE

emergency evacuation and rescue annually and keep the relative record.

3.3 Office all other departments

a) Ensure department staff obey office QHSE management requirement.

b) Manage office except area of supply chain department.

4.1 Office code of conduct

4.1.1 People falling down

Falling down is one of main office hurts, the below code of conduct should be followed::

a) Avoid slippery floor caused by water, snow, etc.

b) Avoid uneven floor, protruding threshold, temporary cables and unexpected odds and ends

to prevent falling down;

c) Use handrails when go up and down stair, avoid fast speed(running);

d) Avoid use improper tool to get something on heights, for example, use chair with wheels

(or office furniture) to get something on heights.

4.1.2 Objects falling down

a) Prevent files and office essentials stored at heights to fall down.

b) Keep the objects hung on wall or ceiling in stable way, prevent the objects to fall down by

poor installation, corrosion, wind, etc.

c) Prevent other objects to fall down.

4.1.3 Fire

a) Use suitable electrical instrument, avoid overload and electrical equipment with defect.

b) Only smoke in designated area.

c) Avoid fire cause by electrical equipment aging.

4.1.4 Cut injury

a) Prevent cutting when use office tools such as cutters and scissors.

b) Paper

c) Other sharp or keen sides and edges.

4.1.5 Crush injury

a) Prevent the fingers to be caught between parts of office cabinet and door, etc.

b) Move office furniture and other items.

4.1.6 Muscle strain and tension

a) Avoid over repetitive motion.

b) Work with proper posture.

c) Use suitable work station.

d) Use proper way and posture to carry something

4.1.7 Electrical shock

a) Use electricity properly.

4.2 Avoid electrical leakage from equipment.

4.3 Office housekeeping

4.2.1 Unsafe factors in office

Poor and unstandardized housekeeping may result in unsafe factors easily, e.g.:

4.2.2 Office housekeeping requirement

4.2.2.1 Office floor housekeeping

a) If any oil spillage or other discharges, water should be cleaned timely, especially about the

walkway, evacuation route and place always occupied by people.

b) Set up warning sign in raining and snowing day or in condition of slippery floor and take

measures to prevent falling.

c) If any tripping hazards, the correction should be made immediately,such as:protruding

pipeline, leg on ground. Temporary cable, positioning of operation material and equipment

improperly.

d) Evacuation exit should be clear with enough lightening.

e) Any opening or hole on ground should be protected properly, such as cover and guardrail,

etc.

f) Do not make cleaning nearby any rotating part such as rotating shaft, moving conveyer.

g) Clean the ground before leaving office or after task completion.

h) Do not cook in dormitory.

4.2.2.2 Waste/ Rubbish

a) Office trash should be classified collection and handling and labelling properly.

b) Domestic refuse and food residue are not allowed to sewer.

c) If the rubbish bin or collector is dirty, use brush and soap to clean and wash it, including its

support cushion.

4.4 Office QHSE requirement

4.3.1 Filing cabinet and work station surface


To prevent the file cabinets toppling:

a) Only open one drawer each time;

b) Always put the heaviest draw at the bottom;

c) Filing the document from down to up;

d) Close the draw of office desk and cabinet immediately when out of usage, to avoid tripping

or hit;

e) Take handle to close draw and close it slowly to avoid finger crushing;

f) Clear the draws when moving filing cabinet and office desk;

g) Seek help when moving heavy filing cabinet and office desk, to avoid muscle strain,etc.

4.3.2 Office lightening

a) Prevent dazzle to avoid eyes fatigue:Adjust angle of displayer, distance between eyes and

displayer should be 45-60cm, to avoid lights dazzle.

b) Make work with interspersed arrangement properly and avoid staring at the displayer for

long time.

c) Keep proper working posture.

d) Adjust situation and height of chair and keyboard, use mouse with ergonomic compliance,

relax your wrist to maximum extend.

4.3.3 Lighting

a) Some accidents were caused by poor lighting, the minimum lighting requirement is listed

below:

 General office work(Review document, ect.):Intensity of illumination should be no

less than 30 feet candle light(300lux).

 Corridor, stairs and elevator: Intensity of illumination should be no less than:20 feet

candle(200 Lux).

b) Direct or indirect dazzle lighting can make eyes fatigue, generally do not work with face

toward window and do not remove light covering, etc.

c) Provide extra lighting if necessary for working.

4.3.4 Ventilation

a) Office area should have good ventilation. The ventilation system may be natural or

systematic and attention that bad air should not be introduced to the office.

b) Clean and sterile air conditioning system periodically to ensure good air quality.

c) Clear the substance which can make office air pollution quickly,clean the places those

can be easily polluted frequently, such as kitchen, toilet etc.

d) Keep good ventilation of copy machine room.

4.3.5 Office equipment

a) During copier operation,close the lighting cover at first and then make the copying, to

avoid eyes contacting with strong lighting.

b) Carbon powder in cartridge of copier, printer and fax machine is hazardous chemicals,

avoid eyes contacting. Use gloves when replace the cartridge, wash hands and face after the

replacement.

c) Shredder may entangle lose sleeves.

d) Do not repair any broken equipment by yourself, it should be repaired by professionals,

give labelling to the equipment with defect or trouble timely, to avoid use in mistake by others.

e) Power off all office equipment when they are out of use.

4.3.6 Office chair

a) Office chair always brings injury to people,the user should inspect the chair frequently,

find the defect timely, repair and replace it timely.

b) Chair can not be used as tool to climb up to bring something, especially for the chair with

wheels.

c) Do not move office materials and equipment by chair with wheels instead of trolley.

d) Do not move yourself quickly by chair with wheels in your office.

e) Do not pick up anything for side of the chair when you are sitting, to avoid side tipping.

f) Keep a good habit of sitting a chair, grab the chair with your hand then sit down, avoid

sitting on the chair without looking at it, avoid sitting at the side of the chair instead sitting in the

middle of chair.

4.3.7 Office floor

a) Uneven and untidy floor surface of office is one of the main reasons for office accident,

such as poor carpet maintenance, threshold protruding, tile broken, etc. These all can lead to make

people falling and injury.

b) Office floor should be anti-slippery, avoid hard, polishing and no waxing floor.

c) Avoid stairs with extra wear and no anti-slip strip.

d) Set up warning sign when slippery falling hazard is exist.

e) Walk slowly and cautiously on wet and slippery floor, running is prohibited.

4.3.8 Articles storage

a) Article should not be put on aisle, walkway, especially do not be obstacle for fire

evacuation route.

b) Article should be stored properly to avoid turnover, generally put the heavy article at the

bottom and put the light article on top.

c) Company should be equipped with proper ladder to avoid people using chair or other office

furniture to bring something.

d) Flammable chemical and other hazardous chemicals should not be stored in office.

e) No items should be put on windowsill, to avoid breaking window glass and falling outside

window.

f) Flower pot and other items on balcony should be fixed safely, to avoid hit people by falling

due to personal reason or bad weather.

4.3.9 Office fire prevention /fighting and emergency response

a) People should be familiar with functions and usage of all kinds of fire control equipment,

such as portable fire extinguisher, fixed fire control equipment, smoke/fire detection system, etc.

b) People should be familiar with functions and usage of all kinds of emergency response

equipment, such as emergency evacuation map, emergency assembling point, smoke mask,

emergency contact information, first aid kit, emergency lighting system, etc.

c) All fire control emergency equipment and facility should be checked and maintained

regularly.

d) Conduct emergency drill regularly, the drill should be organized by operation unit for

operation office area, the drill for company office area should be organized by supply chain

department.

4.3.10 Elevator

4.3.10.1 Office elevator should be inspected on time.

4.3.10.2 Warning sign and passenger notice should be labelled for office elevator, to show the safety

knowledge to passengers politely. Passenger notice should be simple and comprehensive, warning

sign should be obvious.

4.3.10.3 Elevator emergency aid safety management.

a) For any troubles, inform elevator repair personnel and management personnel at first, if

they can not arrive at 5 minutes, try to rescue passengers in trouble by trained rescue personnel for

different conditions.

b) For any other complicated conditions, ask help from elevator company for rescue. After the

rescue, elevator operator should make rescue record and filing it, to accumulate rescue experience.

4.3.11 Other risks control

a) Pencil and other sharp end item in pen container should be kept with tip down always.

b) Keep the scissors and paper cuter properly when they are not in use, do not keep the sharp

end open or point to user and others.

c) Blade of paper cutter should be taken back to protective handle immediately after usage.

d) Do not put glass/porcelain items on side of table, windowsill and other end of surface, to

avoid falling down to break.

e) Prevent or reduce static electricity by way of grounding or other preventative measures,

especially in place such as office on site and control room, etc.

f) Fire lightening is prohibited in office area.

g) Alcohol is prohibited during work.

h) Employees must block their computer by password before leaving it, to prevent important

information release and result in loss.

i) The carried object should not hinder observations to floor, stairs and the sights of other

people.

j) Establish building entry registration management policy and security policy for site

checking at night, to prevent office items stolen.

k) Make visitor orientation for office QSHE requirements.

l) Report possible computer virus and recent common problem timely, recommend

preventative measures.

m) Clear drinking fountains in time to make sure the potable water in good sanitary conditions.

n) Set and maintain office QHSE marks.

4.5 Office QHSE inspection

Like any other QHSE SOP,necessary audit is one of the key links of effective implementation

assurance,QHSE inspection to work and office area is also very important.

4.4.1 QHSE inspection frequency

Office housekeeping inspection should be an organic part of for site tour of supervisor and

management at all levels,the inspection may be conducted regularly or irregularly.

a) The team of company level should be compromised by relative professionals organized by

company supply chain department.

b) Sit office team should be organized by site leader, compromised by relative professional

like QHSE, electrician and conduct inspection.

4.4.4 Inspection record

Inspection record is filed, tracking and correction by the inspection department.
