SPECIMEN SUBMITTED:
1. One (1) GALIL ACE ASSAULT RIFLE, caliber 5.56 mm with serial number
12345 (marked as (08/13/2021 ACU – 1)
2. Eight (8) Fire Cartridge Cases (marked as B-2, B-3, B-4, B-5, B-6, B-7, and B-8)
3. One (1) PNP Issued Canik TP9 caliber 9mm with serial number TP- 147-T8123 with
Five (5) live ammunition (marked as AAA-1.2, AAA-1.3, AAA-1.4, AAA-1.5 and AAA-
1.6)
To determine the caliber of the firearm used and if it will match to the cartridge cases
that being collected at the crime scene
FINDINGS:
Laboratory examination conducted on the above-mentioned specimen submitted that the eight
(8) pieces of fired cartridge were positive from the fired bullets/slugs recovered from the cadaver of
the victim/s PAT JOMEL CANINO y SAPIERA, JOSHUA ONGUS y ANDACA, and ERWIN
VENTURA y SUMAGPANG located at 16 P. Tuazon St. Barangay Socorro, Cubao Quezon City.
Laboratory examination conducted on the above-mentioned specimen submitted that the One
(1) PNP Issued Canik TP9 caliber 9mm with serial number TP- 147-T8123 with Five (5) live
ammunition was not fired on August 13, 2021 at 16 P. Tuazon St. Barangay Socorro, Cubao
Quezon City.
CONCLUSION:
Based on the above findings the fired bullets and the fired cartridges recovered from the
crime scene is from Caliber 5.56mm.

REMARKS:

MEMORANDUM
2. This pertains to the shooting incident that transpired at about 9:30 a.m of August
13, 2021 at 16 P Tuazon St. Barangay Socorro, Cubao, Quezon City wherein the suspect
was ARJAY CORDERO y UBOS, 41 years old, (DOB-March 14, 1980), male, married,
businessman, and a resident of P. Gomez Residences, Cubao Quezon City armed with
long firearm, shot the victims PAT JOMEL CANINO y SAPIERA, 28 years old (DOB-
January 31, 1993) male, single, a bona fide member of Philippine National Police presently
assigned in this station, and a resident of 143 Gen. Santos Ave. Central Bicutan Taguig City
(deceased); and JOSHUA ONGUS y ANDACA, 22 years (DOB- February 4, 1999), old,
single, College student, and a resident of 226 P. Tuazon St. Socorro, Cubao, Quezon City.
(deceased); and ERWIN VENTURA y SUMAGPANG, 30 years old (DOB-April 2, 1991),
male, married, tricycle driver and a resident at 16 Illinois St. Cubao, Quezon City.
(deceased) at the different parts of their body that resulted to the victims’ death.
3. Requesting your good office to conduct Paraffin Casting and Paraffin Testing
to the suspect AR JAY CORDERO y UBOS and Further request that this station be
furnished a copy of the results of the said examination which will used in investigation
and to support the filling of appropriate case/s in court against the suspects.

Paraffin Test conducted to the suspect resulted that he is positive from Gunpowder
Nitrates.
