Thank you for your interest in joining the Dentistry Student Council! This questionnaire is to
help us get to know you better and for us to be able to decide who amongst the applicants
will be the best fit to join the Student Council. Please answer the questions as honestly as
possible. Answers may be handwritten or printed along with this sheet. You may use more
than one sheet of paper for your answers. Submit all questionnaires as a section, and in PDF
format (labeled with “Section - Student Council Questionnaires”) on Friday, August 20 via

these are essential to being part of the Student Council:

2. Share any previous experiences with leading or being student council member
in 5 sentences.

During junior high school elections, I always run as a representative but because of
me not as famous as others, I lost twice and didn’t get the chance to be on a student council.
That sounds sad but for every school task and project, I am the one and part of one leading
the group. From grade 3-10, I was the class president. During senior high, I moved into a
university and I was overwhelmed at first so I didn’t get the chance to be on the student
council. Eventually, after my classmates knew me, they appointed me to still be the one
leading group tasks.
3. What made you want to join the Dentistry Student Council?

Because I was not given the chance to be part of the student council at my past
schools, I want to try joining the Dentistry Student Council. I want to show off my skills and
I want to test my time management skills.

4. What are the qualities that you feel a council member should possess?

A council member must have to be these two things. First, a council member must be
responsible enough to know how to manage his/her time. And lastly, knows how to
communicate with her co-members and with the students.

5. What can you contribute to the Council? In what way would you be an asset to
the group?

During junior high school face-to-face classes, our school hosted a lot of events and
for a year, I observed my seniors how they organized these events. From grade 9-10, I opted
to join my seniors and help them organize school events and I find it really fun. So I think
what I can contribute to the council is to help organize school events.

6. Are you a leader or a follower? Explain.

I am both and play both roles. I believe that a good leader is also a good follower. I
enjoy being a leader and I think it’s what I’m best at naturally. However, I also look for
opportunities to learn and follow those who have more experience than me. I love asking
questions to learn from my managers and more senior team members, and I love having a
chance to collaborate with others. I’ve found that’s a great way to learn. So I enjoy leading,
but I also love working with a strong team and having great mentors to follow.

7. What are the possible things that you think will promote growth amongst us
and the entire student body, especially in the new normal?

The art of communication is the language of leadership. I believe for everyone to be


an effective council member, he/she should be a good communicator. In this new normal,
where it is easy to message someone through social media, we should use this online
platform effectively to send announcements and messages to other students.

8. In your opinion, what is a student council?

A Student Council is a group of elected and volunteer students working together with
an adult advisor within the framework of a constitution or bylaws to provide a means for
student expression and assistance in school affairs and activities, give opportunities for
student experience in leadership, and encourage student/faculty/ community relations.
9. If you were a crayon, what color would you be and why?

What is the color when you take the whole pack of crayons, melt them all together
with a lighter, and mold them into a huge super-crayon? Because whatever that color is,
that’s the color that, if not a fitting description of me, I aspire to be - a beautiful mess.
Alternatively, I’d be okay with being a white crayon because I would be useful to those who
identify my purpose.

10. Describe what you feel makes you stand out amongst the other applicants.

I am passionate and I really wanted to try to be a council member for once in my


whole school life. I want the experience and I like to communicate with other people.

Since I am mostly the leader of our school task and projects, I am the one tasked to make
formal letters like consent letters and others. So, the picture attached below is our
transmittal letter from our grade 12 research.
