Dear Parents/Guardians,

Greetings!

The Health Optimizing Physical Education (H.O.P.E) 0 1 - Self-Testing Activities for Grade 11
students aim to set goals, monitor one’s participation in aerobic and muscle- and bone-
strengthening activities and constantly evaluate how well one has integrated this into one’s
personal lifestyle. Meanwhile, Health Optimizing Physical Education (H.O.P.E) 0 3 - Dance for
Grade 12 students aim to lead dance events with proficiency and confidence resulting in
independent pursuit and in influencing others positively.

In line with these competencies, we will include STEM RUN & ZUMBA as the major activity during
the USC Days for Grade 11 and Grade 12 PE classes on October 25-26, 2022.

To divide the whole STEM population, we will be having two (2) batches for easier facilitation:

October 25, 2022 – 6:00 AM – 7:30 AM – Batch 1: Grade 11 students (RUN)

8:00 AM – 9:00 AM – Batch 1: Grade 11 students (ZUMBA)
9:00 AM – 9:30 AM – Awarding & Giving of Certificates
October 26, 2022 – 6:00 AM – 9:00 AM – Batch 2: Grade 12 students (RUN)
8:00 AM – 9:00 AM – Batch 2: Grade 12 students (ZUMBA)
9:00 AM – 9:30 AM – Awarding & Giving of Certificates

rubber shoes
cap/hat (optional)

To ensure the safety of our students, the USC Emergency Response Team will be present.

We are looking forward to hearing your response and getting your support for this academic
activity.

WAIVER

at the University of San Carlos Senior High School Department, do hereby allow him/her to participate
in the STEM RUN 2022 on ___________________.

I understand that his/her attendance will greatly contribute to one's health and wellness. Most
importantly, I am aware of the risks and hazards of his/her participation at this time of the pandemic
considering the possibility of having contact with unvaccinated students in the University of San
Carlos.

I fully agree to waive any responsibility on the part of University of San Carlos in case of any untoward
incident not limited to risks due to Covid-19 exposure that may happen to my son/daughter in the
duration of the event, STEM RUN 2022.

Finally, I acknowledge that I have been sufficiently informed of the risks involved and give my
voluntary consent in signing this waiver. I further understand that the organizer of this event will
enforce the necessary and minimum health standards during the conduct of the said event.
