C: Vanessa
SN: Okay Vanessa, I am going to check your vital signs every 4 hours. Is that okay with
you?
C: Yes
SN: You can just relax.

2. NPI Proper
a. How will you inquire about the other observations you noted upon taking the Vital
Signs of your client?
SN: Vanessa, How do feel around 4 AM? Do you usually feel cold?
C: Yes, I feel cold.
SN: I notice that you usually have low temperature by that time.
SN: (asks the grandmother)
SN: Is Vanessa usually having this lower temperature? Is it normal for her?
GM: Yes because of weather and the fan.
SN: Okay that’s a relief.
b. How will you inquire on their functional health patterns?
- Health Perception and Health Management Pattern: focused on the person’s
perceived level of health and well-being, and on practices for maintaining health
(habits, vices, allergies).
- Nutrition and Metabolism Pattern: Assessment is focused on the pattern of food and
fluid consumption relative to metabolic need.

C: I like adobo, made by my lola.
SN: What else?
C: Oh, and I also like Piritong Talong.
SN: And how about your favorite drinks?
C: I like juice and soft drinks, but I drink a lot of water.
SN: That’s very good of you. Vanessa,, you really have to drink a lot of water, and eat
nutritious foods.
C: Yes, I like to eat vegetables.
SN: That’s right. It is very utritious and will make you smart.
SN: Vanessa, how many times do you go to the CR? I mean, do you usually go after every
meal?
C: Do you mean how many times I poop?
SN: Hahaha yes.
C: 1 time a day
SN: Oh I see, so you poop regularly?
C: Yes
-Activity and Exercise Pattern: Assessment is focused on the activities of daily living
requiring energy expenditure, including self-care activities, exercise, and leisure
activities.

C: But I don’t play often. I am sacred to viruses.
SN: Yes, you are not allowed to go outside, but you have space in front of your house.
C: Yes, that’s where we play with my cousins.

-Sleep and Rest Pattern: Assessment is focused on the person’s sleep, rest, and
relaxation practices.

C: Yes, when my lola said “go to sleep”, we followed her.
