SN: Okay Vanessa, I am going to check your vital signs every 4 hours. Is that okay with
you?
C: Yes

a. How will you inquire about the other observations you noted upon taking the Vital
Signs of your client?
SN: Vanessa, How do feel around 4 AM? Do you usually feel cold?
C: Yes, I feel cold.
SN: I notice that you usually have low temperature by that time.
SN: (asks the grandmother)
SN: Is Vanessa usually having this lower temperature? Is it normal for her?
GM: Yes because of weather and the fan.
SN: Okay that’s a relief.
b. How will you inquire on their functional health patterns?
- Health Perception and Health Management Pattern: focused on the person’s
perceived level of health and well-being, and on practices for maintaining health
(habits, vices, allergies).

SN: (notices that the child always play outside)


SN: Hello Vanessa, while playing, do you feel anything? Allergies?
C: I don’t have any allergies. I enjoyed playing outside. That’s all.
SN: That’s good to know.
C: Oh! What I noticed is that every time I let my sweat dry on my shirt, I get a cough.
SN: Oh I see. Is there anything you noticed about yourself?
C: None, except for that

C: I like adobo, made by my lola.
SN: What else?
C: Oh, and I also like Piritong Talong.
SN: And how about your favorite drinks?
C: I like juice and soft drinks, but I drink a lot of water.
SN: That’s very good of you. Vanessa,, you really have to drink a lot of water, and eat
nutritious foods.
C: Yes, I like to eat vegetables.
SN: That’s right. It is very utritious and will make you smart.

C: Do you mean how many times I poop?
SN: Hahaha yes.

C: Yes

activities.

C: I don’t have a phone.

SN: That’s very good. It can exercise your body and make you strong.
C: But I don’t play often. I am sacred to viruses.
SN: Yes, you are not allowed to go outside, but you have space in front of your house.
C: Yes, that’s where we play with my cousins.
SN: That’s so nice, Yannah!

- Cognition and Perception Pattern: Assessment is focused on the ability to


comprehend and use information and on the sensory functions (pain, orientation
(time, date, and place), memory).

SN: Vanessa,, you are in grade 6, right?


C: Yes
SN: So how about online school?
C: It’s hard. We have a lot of modules and problem solving.
SN: How do you do your modules?
C: I ask for help from my titas and titos when I don’t know the answer.
SN: Do you remember the lessons you read and answered?
C: No, I’m bad at memorization.
SN: Oh I see, but you do your best to remember the lessons, right?
C: Yes, but it was hard.
SN: It’s okay; you can still develop your memorization skills. Don’t give up

C: Sometimes, because my lolo always watches the news.

SN: But do you sleep early?
C: Yes, when my lola said “go to sleep”, we followed her.
SN: So you have a strict grandmother. But you know what? That’s very nice of you to follow
your lola.

- Self-Perception and Self-Concept Pattern: Assessment is focused on the person’s


attitudes toward self, including identity, body image, and sense of self-worth.

SN: How do you feel about yourself?


C: What do you mean?
SN: Do you see yourself beautiful, even like me I have dark skin.
C: Yes. I know I am beautiful even II have this dark skin.
SN: That’s very good attitude of you.

- Roles and Relationships Pattern: Assessment is focused on the person’s roles in the
world and relationships with others.

SN: Do you treat your friends like your brothers or sisters?


C: Yes.
SN: That’s good to hear. You should always treat them like that, so they will treat you the
same way.

- Sexuality and Reproduction Patter: Assessment is focused on the person’s


satisfaction or dissatisfaction with sexuality patterns and reproductive functions.

SN: You don’t have your first period, right?


C: Yes

- Coping and Stress Tolerance Pattern: Assessment is focused on the person’s
perception of stress and on his or her coping strategies.

SN: Every time you got scolded, what do you do?


C: I go to the room and sleep for a while.
SN: What else?
C: Sometimes I drink water, and sleep.
SN: Oh I see

-Values and Belief Pattern: Assessment is focused on the person’s values and beliefs
(including spiritual beliefs), or on the goals that guide his or her choices or decisions.

SN: Do you ask your lola when you want something?


C: Yes, I don’t make choices.
SN: How do feel about it?
C: I am fine with it.
SN: Okay that’s good to hear.
3. Health Teaching
Based on the NPI, provide important health teaching 2 at least 3 patterns you noted
that needs health teaching for that matter.
SN: Based on the data I collected while doing NPI, it’s the coping mechanism that’s need to
have health teaching. As a kid or in your age it is okay to cry, and to say what you feel. Be
open to your grandmother always. Talk to her you have problem, or when you feel hurt
about their words. But if that’s how you comfort yourself, then it’s fine. Just be open to your
grandmother.
C: Okay nurse , thank you for that.
SN: You are always welcome, and don’t hesitate to come to me if something happens.
C: Yes nurse , thank you so much.
