MATERIALS/EQUIPMENT:Gloves, Blanket or cover kit/shroud, Washcloth, Ties and/ tags clean

1. Gather all materials needed.

2. Check time of pronounced death from chart

3. Closes the door/ window/ curtains

4. Allow time for the family to grieve/pray momentarily

5. Inform the supervisor and census department

6. Explain the procedure to the bereaved family

7. Perform hand hygiene technique. Put on clean gloves

8. Remove jewelries and all valuables of the patient.

9. Put to a bag, labeled and returned to the immediate family.

10. Position the patient into supine.

11. Straighten the limbs gently.

12. Close the eyes gently using the hands. Do not use tape to close eyes.

13. Return dentures properly, if patient uses one.


14. Place a rolled-up washcloth under the chin

15. Place a fresh pillow under the patient's head or raise the head of the bed about 30 degrees.

16. Place absorbent under pads/ diapers under the buttocks.

17. Remove catheters and Tubes inserted into the patient. (IV, IFC, NGT)

18. Cover the insertion sites with cotton and plaster

19. Cleanse the face beginning from the eyes.

20. Cleanse the body thoroughly.

21. Put a fresh gown on the patient.

22. Put a tag (indicating Name of Patient, Age, Date and Time of Death) on the left wrist.

23. Put a tag on the right big toe.

24. Cover the patient from head to toe with blanket/ shroud.

25. Place a tag on the ankle area.

26. Facilitate transfer of patient to morgue.

27. Do after care of materials used

28. Perform hand hygiene technique.
