Initial Findings
Irene is a native resident of Barangay San Dimas, Malinao, Aklan. Together with his husband, Rene, they’ve raised their 5
children using the small amount Rene earned from doing jobs for the others. One of their daughters, Jo-ann Tallon Tapic had a
child named Khim Asley Tapic. The father of the child was not present along the pregnancy and the birth of Khim Ashley. After
she gave birth, the family decided to name the child after their surname.
At the present, Irene takes care of the child while Jo-ann has new family and unable to support Khim Ashley’s daily needs and
school expenses..

Initial Agreement
The client agreed to process an update for an additional household member and submit the necessary documents required of
the household.

Assessment
An additional household member application is requested by the client to put into priority the welfare of the children and for a
client to continuously avail of the grants thru following the conditions of the program as Pantawid Pamilya beneficiaries. The
said family is a certified registered and active beneficiary of the program, with eligible 0-18-year-old children, no pending
grievance complaint concerning the eligibility of the household in the area, and not yet graduated in the program.
