Initial Findings

Daisy is a native resident of Barangay San Roque, Malinao, Daisy was the old grantee. She left to work in Manila. She left their
pantawid to her daughter Eva Implica, who were minor that time. Eva is single and no family yet. She is now working in Kalibo as
helper.
Her sister Carmie Implica is living with her mother, Daisy. Carmie has a child named Ace Inamac. The family of Carmie is
currently under the same household of Eva and Daisy in Barangay San Roque, Malinao, Aklan.

Initial Agreement

The client agreed to process an update for an additional household member and submit the necessary documents required of
the household.

Assessment

An additional household member application is requested by the client to put into priority the welfare of the children and for a
client to continuously avail of the grants thru following the conditions of the program as Pantawid Pamilya beneficiaries. The
said family is a certified registered and active beneficiary of the program, with eligible 0-18-year-old children, no pending
grievance complaint concerning the eligibility of the household in the area, and not yet graduated in the program.
