AFFIDAVIT

I, Falcon Marine Exports Ltd. , Balasore situated at NH-5,khantapara,Balasore,odisha,Dist-Balasore

represented through its General Manager Mr.Pravek Pillai, do hereby solemnly affirm and state as
follows:-

1. That I am the deponent of this affidavit.

2. That one Export Health Certificate Vide Health Certificate No: EIA/BBS/2022-23/03222, Issue
Dated: 24/11/2022 was issued by the office of Asst. Director Exports Inspection agency, IDCO
Plot No. 35/a/1, C.I.F, Patia, Bhubaneswar in favour of our organization i.e. Falcon Marine
Exports Ltd.,Balasore which was send to buyer but now the buyers required the change in
container number as per bill of lading.
3. That, further I/We undertake as the certificate are in transit and the details is attached
herewith but to release the container at port & custom we required in lieu health certificate
in the urgent basis
4. I/We hereby confirm you to submit the original health certificate once it reaches to my office
positively at earliest as possible
5. That, this affidavit is required to be produced before the concerned authority of Ass. Director
Exports Inspection Agency, Sub office at Bhubaneswar and Redg. Office at Kolkata, West Bengal
for information and necessary action.
6. That, the facts stated above are true to the best of my knowledge and belief.
