AFFIDAVIT OF LOSS
2. That aside from aforesaid certificate of title, I am also safe keeping the other
pertinent documents and placed in my cabinet inside our house in Brgy. Bonfal
Proper, Bayombong, Nueva Vizcaya. Unfortunately however, our house was
totally gutted by fire due to an electrical circuit problem in the afternoon of
September 26, 2022. As a result of the fire, all of our personal properties inside
the house were destroyed including the Owner’s Duplicate Copy of Transfer
Certificate of Title No. 036-2014002025 and other documents;
3. That the said fire was put off only after several hours later by the joint efforts
of some neighbors and members of the BFP who responded when called for
assistance;
4. That with the foregoing, I hereby declared that the aforesaid Owner’s
Duplicate Copy of Transfer Certificate of Title No. 036-2014002025 and other
document/s are considered lost and beyond recovery;
5. That I certify and confirm that the parcel of land covered by Transfer
Certificate of Title No. 036-2014002025 was never mortgaged, sold, conveyed,
disposed of or encumbered, to any other person or entity. I certify and confirm
further that said document was neither confiscated nor relinquished in favor of
any government entity, agency or authority;
6. I attest to the truth of the foregoing statements and execute this Affidavit of
Loss to support the petition for the issuance of a new owner’s duplicate copy of
Transfer Certificate of Title No. 036-2014002025, as well as for all lawful
intents and purposes it may serve.
7. That I attest to the veracity of the foregoing statements and hereby execute this
affidavit for the purpose of securing another Identification Cards and
document/s in lieu of the lost one.
