AFFIDAVIT FOR

DEMOLITION OF STRUCTURE

I, DAVE PATRICK M. SAÑEZ, of legal age, Filipino citizen with

postal and residence address at No. 100 Medicion 1-A, Imus City,
Cavite, after having been duly sworn in accordance with law do
hereby depose and state:

1. That I am the owner of a parcel of land situated at

Brgy. Medicion 1-D, Imus City, Cavite, Philippines, covered by
Transfer Certificate of Title No. TCT 057-2016021462 and with
Tax Declaration Numbers 239-0014-03485 and 239-0014-
03486;

2. That I give authority for the demolition of the structure on the

above said described property;

3. That I hereby certify that I have fully described the property to

the demolition contractor, and said contractor has complete
knowledge of the structure(s) that are to be demolished.

4. That I furthermore certify there are no mortgages or liens on

said property and that I am the sole owner of said property. If
there are liens, mortgages, or additional owners they have
been given legal notice of this demolition.

5. That I am executing this affidavit to attest to the truth of all

the foregoing statements and for whatever legal purpose it may
serve.
