1 WORK DESCRIPTION

1.1 Introduction
Vessel inspection for the high pressure containments are a mandatory requirement of A&OI
management programme applied in CLJOC, and compliance with Vietnamese legislation.
Pressure Vessels activities including external and internal inspection follows requirement set
by either Class Body (ABS for FPSO Thai Binh VN) or Risk Based Inspection (RBI) Scheme
for all fixed platforms. The RBI scheme is prepared/ revalidated periodically by Independent
Verification Body, endorsed by CuuLong JOC and approved by Vietnam Register for
implementation. Even though for non-class vessel ones, we need to carry out internal
inspection for safe purpose.
To guarantee safe for all entrants, a vessel must be positively isolated all inlet and outlet
nozzles, vessel free/ or acceptable limit of H-C, toxic gases like H2S, CO, Hg, benzene,
NORM when allocable…as well as sufficient oxygen for breathing. All control measures
need to take place like using proper PPE / tools, available rescue team, contingency plan
set up…
People who involves to the task must be well trained, JSA shall be done in advance and
conducted to whole team members for full awareness of the related risks and control
measure as well as contingency plan. Work control system like permit to work, isolation,
confined space entry certificate…, and numbers of related procedures like LO/TO, confined
space entry, etc… available in CLJOC to assist in safety control for the task.
This Workpack is generated to describe what need to be done for inspection of Glycol
Coalescer (MAD-2445) on STV CPP.

Glycol Coalescer (MAD-2445) is a pressure vessel, installed on STV CPP.
It was designed in accordance with ASME VIII Div.1, type vertical with main key
specification below:

1.4 General Inspection Work Scope


1.4.1 Before inspection
 Safety procedures must be complied with (See Section 2.0 SAFETY)
 CPP offshore to do the Risk Assessment / JSA for vessel cleaning, leak testing, electric
shock etc… when issue the Permit to Work
 Shut down of this equipment is required either Plant Shutdown or Unit Shutdown
dependent upon offshore operation team’s requirements, isolate all inlet & outlet
 Install scaffolding for inspection work if required
 Isolation & removal of the vessels from service and drain & vent liquids and gas
 Purging the vessel with nitrogen and finally air is to create a safe environment inside the
vessel.
 Isolate the manual valves to isolate the inspected vessels from any sources of
hazardous materials if applicable
 Check Gas, O2, NORM, MECURY, H2S…ensure safe condition for man entry
 Open vessel flange & supply forced ventilation for circulation air
 Remove filter element and core for man entry.

1.4.2 External inspection


 Coating condition on the vessel body, supports and nozzles: rub off, spalling, bared
metal exposed to atmosphere, quantify of any corrosion, mechanical damage and
photograph for recording.
 Ultrasonic thickness measurement shall be conducted for vessel’s shell and nozzles
 Condition of vessel shell, heads, nozzles and associated welds for sign of cracking,
corrosion. Any present deformation shall be measured.
 Nozzles, gaskets and flanged connections for sign of distortion, leak, contamination,
loses/corroded bolting.
 Vessel name plate: tag name and specifications are clear and correct.
 PSVs: general condition and detail of information on the name plate are correct, date of
testing…
 Condition of other support elements: earth lugs, skirt, support, insulation…
 External visual inspection of shell, head, skirt support and dimension measurement for
the nozzles
Note: External inspection can be flexibly scheduled before, during or after shut down time
when the inspectors are available
1.4.3 Internal inspection
 Internal visual inspection inside vessel such as corrosion conditions, weld condition,
solid debris for element of stopper, adaptor and breaker plates…
 Remove & check filter elements, seal of quick opening closure
 Penetrant testing shall be conducted at accessible weld joints or suspected areas
 Internal inspection for lower part of the vessel and header shall be conducted by
boroscope via hand-hole for general condition of internal surface, welded joint and
fittings.
The service should get information of actual situation of the internal wall, any wear / tear
sign, pitting, corrosion, erosion… All abnormal condition shall be taken measurement details
and report with specific location for further tracking, monitoring (see appendix)
1.4.4 After inspection:
 Vessel final inspection and close hand-hole & opened nozzles
 Re-swing spectacles of the inspected vessels.
 Leak testing for the inspected vessels If required, this is confirmed later on by CPP
Supervisor
 De-isolation.
 Destruction of scaffolding platforms outside vessels.
Inspection reports will be issued and signed off with ABS and CLJOC representative on site.

1.5 Work package plan


The vessel inspection is planned to be carried out and required Unit Shutdown, depends on
operation status, Operation planners (onshore & offshore) will suggest exact date to
implement the inspection.
The scaffolding, isolation, flushing, purging, ventilation and drain need to finish before
starting the inspection.
The total schedule takes around 2 days.

2 PREPARATION

2.1 General Safety/HSE Screening


 Complying with CLJOC HSE policy.
 Manpower must have necessary certificates including safety certificates (BOSIET,
HUET), scaffolding, NDT certificates. The Contractor supervisor checks to ensure
correct certificates for his working party. CLJOC supervisors may random check if
necessary
 Contractor staff is requested to complete the CLJOC pre-going offshore training course
prior to travelling offshore.
 Contractor workers should have PTW level 1 training and supervisors (work leader)
shall have PTW level 2 training.
 Activities carried out at CPP must be controlled by the Permit to Work system, Confine
Space Entry Certificate
 Contractor supervisors shall supply the permanent supervision to ensure that safety is
always maintained for their working party and working place during the job.
 All activities must be done under the supervision of CLJOC authorized persons.
 “Toolbox talk” to working parties should be conducted prior to any jobs
 Personnel must be provided all necessary PPE for work to meet CLJOC safety
requirements. Contractor is responsible to provide PPE for its crew to follow CLJOC
PPE procedure
 Only skill & qualified persons can do the scaffolding, lifting, rigging job.
 All equipment and tools shall be inspected to ensure that they met CLJOC specification
for using at site and/ or workplace as well as hazardous area (if any).
 All electrical equipment is used at CPP shall be suitable to the following power supply:
 450V, 3P, 60Hz and 220V, 1P, 60Hz
 All portable and temporary equipment used shall be complied with the CLJOC
procedure for portable & temporary equipment CLJOC-HSE-PRO-HS-0031 Rev.C
 The Risk Assessment / JSA should be done prior to implementing the work as the
CLJOC-HSE-PRO-HS-0015a/b
 Caution is taken to avoid improper termination, short circuit, insulation damage and
breaking the wire and equipment throughout the installation, testing and commissioning.
Clean up the workplace after completion.

2.4 Scaffolding / rigging / tools


2.4.1 Scaffolding:
Those condensers’ inspection is required to install scaffolding. To access the nozzles at
height for positive isolation and access into the vessel via Inlet/Outlet covers from access
platform
2.4.2 Tools
 Hydraulic torque wench (1): for losing and tightening the manhole bolts
 Boroscope kid (1) for inspection areas which can’t access.
 Light, Torch (non-spark, less than 50V)
 Ultrasonic thickness Gauge, MPI equipment
 Lifting gears, hand tools: combination spanners, torque wenches, flange align bars,
multiple socket sizes etc...
 Air blower + 2 off flexible ducting (5-10m) if needed
 Other special tools / equipment and materials required for the internal inspection

2.5 Manpower / Role & Responsibilities


2.5.1 Manpower:
Below is recommendation list of people who should involve to this task. Note that they all
won’t just only working on this vessel but others during the shutdown, the quantity will be
considered by offshore and put in an integrated shut down plan.

Operations support team has responsibility for arrangement contractor of fitters, scaffolder

3.3.1 Role & Responsibilities
 Surveyor / A&OI engineer: responsibility for witness the inspection and take note of
current condition of the vessel
 Inspector: responsible for external & internal inspection of vessel, record actual
condition into report form
 M& R technicians / fitter contractor: responsibility for spading and de-spading, replace
anode bar, and other mechanical work. He is to ensure all required materials, tools
available at site before the shut down
 NDT contractor: responsibility for thickness measurement, and examination crack on
materials, welding by MPI

 Leak test personnel – P&P: responsibility for purging and leak test the vessel breaking
points where the joints are de-bolted and re-bolted (positive isolation points). Their
scope included is to safety for their equipment during operating period
 Vessel cleaner: responsibility for internal cleaning the vessel until satisfaction for
inspection.
 P&P / CRO: responsibility for shutdown and restart-up process, carrying-out isolation,
de-isolation, flushing, purging, ventilation
 Scaffolder: Responsibility for erecting and removal the scaffolding outside the vessel.
 H-C tester: Responsibility for checking H-C, toxic gas, Hg, Benzene, and NORM inside
the vessel.

3 IMPLEMENTATION

3.1 Onshore work


 Materials preparing, manpower contracts / call off preparation.
 Rigger/fitter and cleaners will be called out from awarded Contractors and then
mobilised to CPP when starting job.
 Materials required for the isolation / de-isolation jobs will be sent to CPP before the
inspection job for isolation / de-isolation works & opening / closing the manhole, swing
spectacles, open/close nozzles

3.2 Offshore work


3.2.1 PREPARATION JOB
 Carry out Shutdown of all equipment connection to vessel
 Isolate all main inlet/ outlet manual valves of vessel and isolate other lines/ nozzles if
necessary (refer to Isolation, Depressurization, draining/purging plan drawing)
 Carry out draining, purging as remarks on the job steps (refer Isolation,
Depressurization, draining & purging job steps)
 Remove instruments, piping connections if required
 Check LEL and then purge for vessels by N2 or inert gas is required prior to inspection
 Open & check quick opening closure
 Remove & check internal parts (as filters, elements…)
 Install scaffolding, temporary platform for inspection work,
 Carry out the cleaning by using high pressure source is N2 or inert gas
3.2.2 INSPECTION JOB
 Internal and External inspection: The inspection will be conducted by VR, IVB & A&OI
engineer.
 Visual check the external/ internal coating, welded joints, flange joints of vessel
 Perform shell thickness measurement

 NDT inspection: NDT method will be applied for detect the associated with suspecting
damage mechanism.
 Perform boroscope inspection for lower part of vessel by offshore M&R engineer.
 Complete the inspection job
 Inspect Filter Elements and replace by new one if found damaged
3.2.3 AFTER INSPECTION
 Reinstall internal parts & using new gaskets in the same orientation as when they were
removed and note the position of the nozzles.
 Re-install the removed instruments, piping connections if any
 Re-swing spectacles if needed
 De-isolation job
 Leak test if necessary
 Check service in line with Operation Manual
 Housekeeping job
 Inspection job completed

3.3 Breaking Containment


The detailed sequences of isolation, depressurization, draining and purging, refer to Job
step plan for isolating, depressurizing, draining, purging for vessel inspection as attached.
Boundary isolation request for this task is fully isolating of the inspected vessels. Please
refer drawings below for guiding isolation, draining and purging.
3.3.1 System isolation
Due to Glycol coalescer (MAD-2445) on the same Gas Dehydration system can be shut-
down at the same time. Therefore, boundary isolation can be performed for the whole group
and unit isolation can be arranged reasonable for combination in venting, draining, flushing,
purging …that is flexibility in implementation by field production technicians who are
qualified performers.
Purging for this vessel is required to ensure free off H-C before breaking containment
purging process also need to be applied in this case. Purging route is as mark-up on
drawings
3.3.2 System depressurize and drain
The depressurization and draining of vessels are required to carry out prior to commence of
internal inspection activities. Please refer the attached plan of depressurize & drain.
Carry out depressurize vessels as remarked on depressurization & draining plan & Job
steps.
3.3.3 Positive isolation
Because the vessel will be do unit shutdown for internal inspection so that the isolation
procedure shall be carried out sequences of steps as per attached positive Isolation plan.
3.3.4 Air ventilation
After opening hand-hole of vessel, let them natural air freely gets into the vessel, then
positive ventilation to be arranged, air supply by blower at hand-hole and comes out at quick
opening closure of vessels. Air ventilation about 30 minutes then stop for gas test inside the
vessel and repeat until getting satisfactory. Air must be continuously flowed during time man
entry in the vessel with low speed.
3.3.5 Confined space testing requirement
Before cleaning, the vessel must be tested for sufficiency of O 2. Nobody reach close the
vessel (including the testers) when the authorized tester signs off the certificate to confirm
the vessel is in safe condition for internal cleaning and inspection, then the permit to work
issue for working in the vessel.
3.3.6 Vessel internal cleaning
Depend on how dirty of the internal parts, shell and bottom of the vessel, cleaning can be
arranged by flushing fresh water.
After removing Filter Elements out of vessel, Vessel will be cleaned by fresh water or air.
3.3.7 PPE requirement
Apart from normal PPE requirement, in case of the man who entry if required to the vessel
shall wear extra following things:
 Chemical protective clothing, hooded coverall
 Chemical protective aspirator.

3.4 Internal Inspection


3.4.1 Safety requirement for vessel inspection
PTW only issues when the Authorized gas tester completion the test of Oxygen and
confirmation safe for internal inspection.
It is recommended CLJOC Confined Space Entry procedure (CLJOC-HSE-PRO-HS-0012)
must be strictly complied.
3.4.2 Vessel internal inspection
The following inspection activities shall be conducted mandatorily after complete cleaning,
purging.
 Remove internal parts (filters & accessories) through quick opening closure & ensure
not damage to them.
 Check internal parts for any sign of build-up of surfaces and clean surfaces if found
build-up.
 Check internal/ external coating of vessel
 Visual check welding joints of vessel
 Boroscope for lower part of the vessel
 Check enclosure gasket, seal of hand-hole, quick opening closure, nozzles and replace
if damaged
 Reinstall internal parts using a new gasket and taking care not to damage the parts.
Internal parts must be reinstalled in the same orientation, location as when it was
removed
 Check for loose connections and all wiring for signs of insulation breakdown or
brittleness (If required)

3.5 Re-Installation
3.5.1 Removal positive isolations
When completion of internal inspection, to ensure all tools, auxiliary devices, torch etc… are
cleared out of the vessel before reinstallation of internal parts, then removal of the positive
isolation.
Replacing all gaskets, with correct torque and bolt tighten sequence are requirement for
each flange.
3.5.2 4.5.2 Oxygen Elimination
After reinstatement, N2 purging will be conducted to eliminate oxygen from the vessel and
pipeline. This shall be done to get rid of O2 by sweeping N2 before leak test. Sweeping can
be done by pressurizing vessel up to 3 barg with N2 then depressurizing during 2-3 cycles to
check O2 less than 5%.

3.5.3 Leak test


Leak test can be carried out at 7 barg as existing nitrogen system then increase pressure to
141 barg of MAD-2445 by N2 bundles.
Visual check and snoop liquid detector are used for detection leak on the breaking joints.
Holding time will keep in 30 minutes for monitoring any drops pressure.
