Good day! I hope this letter finds you well!

Cebu City Medical Center has been known to its dedicated and exceptional service to the people
of Cebu City. Part of how we serve with confidence and grit is by making sure we look professional and
trust-worthy.

With this, the Nursing Service Grooming and Uniform Committee is respectfully submitting this
proposal in line with the General Uniform Guidelines, the retention of our current TYPE A design (See
attachment A to E). Each staff shall be required of two (2) new uniform sets done by their own
seamstress or tailors of choice following the prescribed designed and cut. To confirm this, the uniform
sets will be checked and inspected by the staff’s Senior Nurse on its given deadline. This will also be
validated by the supervisors during their rounds.

If found out that the staff is still wearing the old ones, Anecdotal sanction shall be served duly.
Compliance of all the staff under the Nursing Service Department on this will be ensured.
