Kindly fill up the information under Annex “A” and Annex “B”
as required in Memorandum No. 68 on or before two days before the
licensure examinations. Printed copies of Annex “A” and “B” will no
longer be collected during the examinations. Kindly present
screenshots of your submitted Annex “A” and “B” to the
Examination Proctor upon entry in your designated examination room.

Kindly present any one of the three: 1) RT PCR results 2) Certificate of

Quarantine or 3) Vaccination Card, to the Room watcher upon entry. Be advised
to bring your lunch and snacks.
