SECURITY GUIDE - PERSONNEL ENTRY, EXIT & RE-ENTRY FROM PROJECTS AND QUARTERS

RELATIVE TO COVID-19 PREVENTION & MANAGEMENT INSPECTION CHECKLIST

A. Recording

1 PHR shall maintain over-all personnel
inventory on Quarters including temporary
facilities
located inside offices and warehouses.
2 Vendor representative shall also maintain list
of company Personnel housed in MDC
Quarters,
offices and warehouses.
3 The following information shall be
maintained:
a) Company Name
b) Name of Personnel
c) Age / Gender
d) Place of Residence including Barangay
e) Skill / Position
f) Date of Confinement to Quarters
g) Immediate superior
h) Quarters location description
i) Person and phone number to contact in case
of emergency
4 The name of the company and Personnel
residing in each Quarters shall be posted at
the
entrance door for easier identification when
contact tracing or during an emergency
5 All information pertaining to Quarters’
occupants shall be shared with CMHS and EHS
B. Quarters Entry, Exit and Re-entry Requirements for Works

1 Entry, Exit and Re-entry Requirements
a) PIC or PM identifies personnel to reside in
Quarters and notifies PHR
b) Offsite and Near-site Quarters entry / exit
gate passes shall be different than another
gate
passes indicated in Section III.C of the
guidelines
c) All entry / exit gate passes shall be
approved by the GM.
d) PHR shall issue an entry gate pass once
approved to each personnel.
e) PHR shall furnish the Security Gate Guards
at the project and Quarters a list of personnel
issued with an approved entry / exit gate pass.
f) All personnel shall present the approved
entry / exit gate pass to the Security Gate
Guard
upon entry and exit
g) The Security Gate Guard shall use the list in
counterchecking personnel on entry, exit and
re-entry to and from project and Quarters for
work reasons along with checking personnel
ID.
h) The Security Gate Guard shall be
responsible to check the temperature of all
personnel
entering and exiting the project and Quarters.
i) If a high temperature reading is detected
the Security Gate Guard is to immediately
notify CMHS and isolate the person in the
Temporary Quarantine Chamber (TQC) until
health assessment is conducted.
j) Projects using offsite or near-site Quarters
shall be required to submit a plan to the
OpCen
GM for approval on managing the travel
between project and Quarters to prevent
personnel
from interacting with the general community.
2 Exit and Re-entry for Non-work Requirements
a) When leaving the Quarters for personal
leave reasons and returning, all personnel
shall follow the requirements of section
III.C of the guidelines
b) Personnel staying in Quarters are not
authorized to run errands or to purchase
food, etc. outside the project and returning
as this is breach of the Protocols
C. Quarters

1 Initial Entry and Re-entry Requirements;
a. All personnel entering the Quarters for
work purposes shall have undergone CMHS
CDF and holding/quarantine period and
COVID testing requirements (based on
latest CMHS entry requirements algorithm)
before being issued a Fit-To-Work (FTW)
clearance by CMHS
b.Once given FTW clearance by CMHS all
personnel shall be validated by PHR and
approved by the OpCen GM before being
issued an entry gate pass.
c. PHR shall furnish the Security Gate Guards
at the project a list of personnel issued with
an approved entry gate pass.
d.All personnel shall present the approved
entry gate pass to the security gate guard
upon entry.
e. The Security Gate Guard shall not allow
entry of personnel without the approved
entry gate pass.
f. The Security Guard shall keep the entry gate
pass after validating and checking
temperature and then give to PHR.
g. For initial entry the cost of quarantine
facilities and testing shall be at the expense
of the employer.
h.Personnel is expected to be CDF-screened
at regular intervals while at the quarters.
i. Cost of PCR testing for MDC employees, if
less than 60 days interval, shall be for the
account of the personnel unless authorized
by the General Manager or Group Head.
2 Issuance of Exit Gate Pass for Personnel to
Leave the Project for Personal Reasons;
a. Personnel apply for an exit gate pass
through PHR.
b. PHR shall issue a gate pass if the exit from
the project has been approved by the
OpCen GM or Group Head.
c. PHR shall furnish the Security Gate Guards
at the project a list of personnel issued with
an approved exit gate pass.
d. All personnel shall present the approved
exit gate pass to the Security Gate Guard
upon exit.
e. The Security Gate Guard shall use the list in
counterchecking personnel on exit along
with checking personnel ID.
The Security Gate Guard shall record details
of all personnel who exit the project and
provide; a copy of these details daily to PHR.
The details required are:
i. The person’s name
ii. Whether issued with approved exit gate
pass or not
iii. Employer
iv. Position
v. Exit date and time, then issue those
details to PHR the same day.
3 Leaving the Quarters without an approved
request and non-issuance of ‘Pass”;
a. Personnel who wish to exit from the
project who have not received approval
from the GM and have not been given an
exit gate pass cannot be stopped from
exiting but the Security Guard shall
immediately notify PHR and shall be
recorded as indicated above.
b. All personnel found to have left the
Quarters without an approved request and
that was not issued an official “Exit Gate
Pass” shall be required to cover all costs
associated with mandatory quarantine
facilities and COVID testing prior to being
issued with an “Entry Gate Pass”.
D. Eligibility to exit the Project

1 Personnel who have spent 60 consecutive days
stay at the project in Quarters
2 Personnel with emergency situations that require
their physical presence
3 Family matters that require their physical
presence
4 Personnel on item D.2 and D.3 shall be subjected
to re-entry procedures, the cost of which shall be
for the account of the personnel.
E. Frequency

1 all personnel shall be issued
with exit gate pass to attend to personal
matters outside project premises only once
a month.
2 In case of an extreme emergency situation,
personnel maybe issued pass to go out of
project
premises subject to recommendation by the
PM/PIC and final approval by the GM or
COG Head.
