HIRA (Hazard Identification and Risk Assessment)


1- PURPOSE:

The purpose of this Method Statement is to describe the HSE and quality control
Procedure to be followed during the BLOCK Works for Construction and operate phase-1 green
Riyadh nursery project.

2- SCOPE:

The scope of this Method Statement is to define all the procedures and inspections to be
followed during BLOCK WORKS on this Project, in compliance with the project specifications
and standards procedures.

5- RESPONSIBILITIES

5.1 Project Manager

 Ensures that the content of this method statement is understood and implemented at all
times.
 Coordinate with the site engineer to ensure that the necessary resources to implement
the approved Method Statement have been allocated to the task.
 Determine all the requirements needed in order to comply with safety standards and
regulations as require.
 The progressing works are carried out according to the planned program and all the
equipment’s required to execute the works are available as per the project planning
and are in good condition
 Co-ordinate with the Consultant, Project Manager, Site Engineer, Safety Engineer, and
sub-contractor for the safe and proper execution of the work.
5.2 QA/QC Engineer.

 All inspection and testing as identified in ITP will be conducted, QC check sheets as
indicated in the ITP will be completed.
 Will ensure that the site block work activities are conducted according to the approved
Method Statement and project contract documents.
 Shall be responsible of the implementation of this procedure.
 Will ensure that the materials being delivered and used are in accordance with the project
specifications and approved material submittal.
 Shall be responsible for the inspection, preparation of all documents and compiling them
for the inclusion in the final documentation packaged.

5.3 HSE Manger

 The HSE manager is responsible for reviewing this procedure to ensure that systems of
work is in place/considered in each phase of activities.
 He also has the overall responsibility of ensuring, through the safety supervisors/officers
that the works in the field are performed safely and in compliance with all applicable
quality standards stipulations and procedures of Royal Commissioning of Riyadh City.
 The HSE manager reports and coordinates all HSE matters with Royal Commissioning of
Riyadh city HSE team along with the functional responsibility to the project manager.
 Ensure proper welfare arrangements for all manpower and staff

5.4 safety officer/Supervisor

 Will ensure that all the activities related to the specified task are being followed safely and
all workers are wearing PPE.
 Identify and take measure control the risks associated with the hazard.
 Ensure that the site is maintained clean and tidy.
 Inspect all the site activities and train the persons to prevent accidents and its proper
reporting to the project manager and the client.
 Ensure all required documents (ITP, approved MS, RA, checklists, 3rd party certificates,
etc.) are available and up to date.
 Ensure all staff and workforce involve in the job are aware about the emergency plan
 Shall coordinate directly with the project/site engineer in regards to advisement of safety
issues and problems.
 Report any violations to HSE manager in order to take corrective action.
 Ensure that the site is maintained clean and tidy.

5.5 Site Engineer

 It will be ensured at all time that BLOCK works shall be carried out under the control of
competent works supervisor. Assigned supervisor shall have extensive experience block
works undertaken
 Shall explain the Method Statement for his subordinates and responsible for executing the
works in accordance with the method statement.
 To ascertain that the work is being carried out as per the instructions of site engineer and
in conformance with the approved drawings & specs.
 Also to instruct workers to eliminate hazards and encourage safety actions or Initiatives.
 Ensure all materials/ equipment’s to be used is available prior to start of work
 Co-ordinate with the QC engineer for the tests to be carried out and initiate for the
inspection for the finished works.
 Ensure that the site is maintained clean safe and tidy

5.6 Foreman

 Carrying out the work and the proper distribution of all the available resources in
coordination with the site engineer on a daily basis.
 Daily reports of the works achieved and coordinate for the future planning with the Site
engineer.
 Incorporate all the QC and HSE requirements as requested by the concerned Engineer.
 Met with any type of unforeseen incidents or requirements and reporting to the site
engineer immediately.
 Reports directly to site engineer.

5.7 Site Survey Engineer

 It will be ensured at all time that surveying works shall be carried out under the control of
competent surveyor.
 To ascertain that the work is being carried out as per the instructions of site surveying
engineer and in conformance with the Plot Co-ordinate of Phase-1 green Riyadh Nursery
project provided by Royal Commissioning for Riyadh City.

5.8 Precaution to Avoid Corona Virus COVID-19

1. Clean your hands often


2. Wash your hands often with soap and water for at least 20 seconds, or after blowing your
3 nose, coughing, or sneezing.
4. If soap and water are not readily available, use a hand sanitizer that contains at least 60%
Alcohol. Cover all surfaces of your hands and rub them together until they feel dry.
5. Avoid touching your eyes, nose, and mouth with unwashed hands.
6. Avoid close contact
7. Avoid close contact with people who are sick

5.9 Social Distancing:

Social distancing means consciously increasing the physical space between yourself and
Others.
These are measures you take to limit the spread of COVID - 19.
1. Keep at least 1 to 2 meters
2. Stay away from a sick person
3. Call MOH online services
4. Stay at home when u sick

7- SAFETY PRECAUTIONS:

1) Task specific safety briefing / tool box talk has been conducted by HSE dept. prior to
start of work activity.
2) Safety officer and direct supervisor will ensure that PPE are worn by site personnel
at all times during this activity.
3) Safety officer shall ensure that all workers and workplaces are safe by continuous
monitoring and providing safety instructions.
4) All personnel must adhere to the safety rules and regulations at the site.
5) Hazard identification and control measures has been implemented.
6) Only approved scaffolds should be used
7) Proper housekeeping is done on a daily basis.
8) Safety warning signs, lights and notices shall be displayed as needed on site
locations.
9) Excavations / Permits shall comply as per HSE plan.
10) It will be ensured that the equipment used at site shall be maintained and relevant
certification (including operator training certification and / or licenses) shall be
Submitted.
11) Housekeeping shall be given prime importance and carried out on a regular basis.
Prevention of entry by personnel into restricted areas, and a high regard for the
environment and natural resources shall be adhered to at all times
12) Experienced banksman will be assigned for machinery operation.

Coordination with other Disciplines


To ensure that there is no aborted/defective works during the execution of block work. It
ensures that the latest information related to any discipline is incorporated during block work
activity and all the relevant disciplines are informed to ensure effective coordination and
smooth interface when the stage reaches for the relevant discipline.

8- PROCEDURES / METHOD

1) Blocks shall be fully wetted before being placed.


2) Surveyor will provide setting out as per approved drawings with reference to the grid lines.
3) Once the setting out is marked first layer of block shall be laid according to the approved
block work plan and for superstructure walls first layer of hollow blocks shall be filled with
mortar.
4) The starter course shall be thoroughly checked and approved by the consultant including
location of door opening and control joint if any.
5) Once the setting out is approved shall proceed further block work according to the
approved shop drawings and specification.
6) The mortar shall be mixed in proportion 1:3 and shall be mixed with the maximum
allowable water to compensate for the absorption of water by the blocks from the mortar.
7) Hardened mortar shall not be used in the work and shall be removed from the site.
8) Mortar joints will be maintained with a maximum 12mm thick with uniform pattern.
9) The connection between RCC elements and masonry will be done by using steel ties and
masonry reinforcement and shall be as per the approved drawing.
10) All internal and external angles shall be kept strictly in true line, level and angle.
11) Shall interrupt joint reinforcement at control and expansion joints.
12) Line and level shall be surveyed during the process of laying of blocks.
13) All door and window openings shall be made according to approved block work plans and
block cells around opening shall be filled with grout.
14) Concrete lintels shall be provided on all openings above 400mm and shall be as per the
structural specifications.
15) Seven courses of block will be placed at a time unless prior approval of consultant is
acquired.
16) Bond Beams will be provided as per the approved drawing wherever required.
17) Alternate courses to be interlocked and joint reinforcement shall be lapped at
intersections.
18) Masonry units shall be cut with motor driven saws to provide clean, sharp, unchipped
edges and to provide continuous pattern.
19) Chasing work will be carried out taking note of chasing requirements and limitations.
20) Mortar joints of all wall to receive plaster shall be racked out to a depth of 15mm.
21) The top of the non-load bearing wall shall be finished with a layer of 10mm of easily
compressible material between the wall and the soffit of the load carrying beam/slab.as
per the approved drawing
22) Curing of block work will take place for two days by spraying potable water on the erected
walls.
23) Mortar specimens shall be tested according to the project requirement and test result
shall be submitted to the consultant.
24) Post block work inspection shall be carried out prior to the next task.
