RELEASE, WAIVER AND QUITCLAIM

KNOW ALL MEN BY THESE PRESENTS:

That I, ____________________________, of legal age, Filipino, and a resident of

___________________________________, state that I have voluntarily booked, used,
and availed of the premises and amenities of xxx Farm on _____________, 2021.

I hereby release and discharge xxx Farm, employees and staff, from any and all
claims associated with my voluntarily booking, use, and availment of the premises and
amenities of xxx Farm, including, but not limited to, anything that would result from the use
of the pool or other amenities of the farm, and all such risks being known and appreciated
by me, and I personally assume all possible risks attendant thereto.

Moreover, I agree to strictly observe and comply with all the safety quarantine
protocols contained in the various Municipal Ordinances issued by the LGU of Bacolor,
Pampanga, as well as the several IATF Resolutions and DOT issuances. These shall
include but not limited to the mandatory wearing of face masks and face shields in public
places, the compliance with the maximum allowable 50% venue capacity, the social
distancing requirement, the new normal safety guidelines, and the like.

I also acknowledge that I am holding my event/activity during the time of the

COVID-19 pandemic, which involves certain inherent risks, dangers and hazards, and
possible exposure to and illness from COVID-19. I hereby freely agree to assume and
accept all known and unknown risks of exposure to COVID-19,and assume full
responsibility for our participation in the event. I likewise recognize that while protocols and
personal discipline may reduce this risk, the risk of serious injury, illness, and even death
is not possible to fully mitigate.

In addition, I also represent that my companions and I are in good health and have
had no known exposure to COVID-19 and no symptoms of COVID-19, for 14 days prior to
attending the event.  

Finally, I declare that I have read this document and the release, waiver and
quitclaim hereby given are made willingly and voluntarily, and its contents were explained
to me in the language and dialect I understand, with full knowledge of my rights under the
law.
