II. Project Description

Our office would like to propose “BAKUNADO PANALO” raffle which aims to incentivize
vaccination against COVID-19 in the bid to increase vaccine willingness in the municipality. This is
in pursuant to the Presidential directive pertaining to implementing COVID-19 strategies to achieve
90% primary series coverage among Priority Group A2 (Senior Citizens) and 50% first booster
coverage among the general population.
Through this raffle, we celebrate the contribution that each Filipino makes in choosing to get
vaccinated and playing their part to end this pandemic.
III. Mechanics
A. How to join
a. The activity period runs from September 12, 2022 to December 9, 2022
b. Only those individuals who will get vaccinated during the activity period are entitled
to one raffle coupon.
c. The vaccinee must completely fill out raffle coupon with his/her complete name,
address and cellphone number
d. A designated drop box will be made available in all vaccination sites within the
municipality where the vaccinees may drop their raffle coupon
B. The Grand Raffle Draw
a. The grand raffle draw will be held on December 9, 2022 at Calayan Municipal Hall
located at Castillejos St. Poblacion Calayan
b. 1 Grand Winner and 412 Consolation Winners will be drawn to win below prizes
i. 360 winners of 500.00
ii. 50 winners of 1,000.00
iii. 2 winners of 5,000.00
iv. 1 winner of 10,000.00
C. Claiming the Prize
a. Winners must present the either of the following in order to claim their prizes;
i. One Valid ID with Picture
ii. Text message from MHO

II. Project Rationale & Objectives
Rest and Recuperation is a means by which staff members can take an authorized time off to leave the duty
station approved for rest and recuperation activities in order to give recognition for their incomparable rendered
services.
It is undeniable truth that the success of organizations and employees is directly correlated to the successful,
effective, and efficient organization and functionality of cohesive inter-disciplinary and inter-collaborative
teams.
Team building increases employees’ productivity as well enhances their motivation. It also encourages
collaboration while also fostering trust and respect among employees. People can make genuine connections
with the help of this activities and assist people in learning how to collaborate effectively and get along well
with each other.
In order to encourage employees to interact with one another in non-work-related environment, team building
exercises should be held outside of the office. These activities should also provide a reason for the employees to
collaborate in order to solve a problem, make them feel valued and concurrently improve their relationship.

accomplished throughout the year.
