Therefore, we propose , that you file/process your early retirement with the GSIS. You can
avail of the Eighteen Months lumpsum plus the money value of your Insurance Policy and the
monthly pension at age 60. Your advantage will be within four years only, because the GSIS
policy forfeits your benefits not claimed and remained dormant beyond the prescribed period.

Madam:
Marigondon National High School is a big school consisting of One Hundred Twenty
(120) teachers with an enrolment rate of 3,379 students and attached schools namely: Gun-ob
High School which has a population of 1,087 students; Basak Night High School, 855; Gun-ob
Night High School, 300; Sudtonggan Night High School, 300 and STEC, 113 students.
This Office proposed for reclassification, six (6) Teacher III Items to Head Teachers III,
two ( 2 ) for each subject in Science, Math and Filipino in consideration that one (1) Head
Teacher III of each subject will be retained in the mother school in Marigondon National High
School and the other three (3) who are also passers of the Principal’s Examination will be
detailed to its attached schools with greater enrolment probably in Gun-ob High School, Basak
Night High School and Gun-ob Night High School.
With this representation, our Office hopes for the approval of the above request.

Consistent with the provisions of the CSC Omnibus Rules on Appointments and other
personnel Actions, this office hereby prescribes the following guidelines ( see enclosure) to
govern the processing of appointments to vacant positions and anticipated vacancies as a result
of chain promotion. This is done to ensure that said vacancies shall be filled up expeditiously.
You are hereby requested to coordinate with retirable employees (aged 60 and above) in
your respective schools and advice the latter to file their retirement papers at least one month

we do not have the List of Scholars wherein we can pick out the name to fill in one item
projected for DOST scholars.
The allocation of teaching positions for the first batch of 2011 had already been filled up
and deployed to recipient schools based on the deployment report, but immediate action to
prepare the authority to fill for the three items of the second batch is still awaiting for the release
of the NOSCA from the DBM.
The filling up of new positions will be done appropriately upon receipt of the NOSCA.

reclassified Secondary School Teacher Items to Head Teacher III, who are not acting as
administrators, whether or not they have to report to school on summer vacation.
We have also other concern about their leave credits. If they are not to report to school on
summer vacation, will they adhere to the teachers vacation service credits or avail of the
vacation and sick leave credits as other non-teaching are enjoying, considering that Head
Teacher position falls on the non-teaching group.
May we have your opinion, as this office stands by your policies and guidelines relative
thereto.

1. This office has prepared the List of Names and assignment of teachers to occupy
the 22 newly created Teacher I Items for the elementary and 52 newly created Teacher I
Items for the secondary, based on the deployment report of the proposed positions in our
Division and the NOSCA approved by the DBM, received by this office on June 3, 2011.
2. We have submitted to your office our request for authority to fill on June 30, 2011 with
the names of teachers, strictly observing the ranking in the RQA, and proper consultation
with the local government, yet we cannot issue their advice pending approval of the
same.
3. As such we also cannot submit the profile of teachers as required unless they submit their
documents for appointment which can only be done if we give them offer to teach.

Series of emails to GSIS Manila have been made since I retired last January 1, 2008,
regarding the deduction of P103,099.20 for alleged salary loan balance deducted from my
lumpsum annuity.
Just this July of 2011, I received an email from Mitos G. Galicano informing me that I
have a refund of P6,612.45 dated June 25, 2011. Per supporting documents I submitted, I am not
amenable of the amount since my last loan is only P50,000 of which my payments per payslips
totaled up to P57,386.53.
In this regard, I would like to ask you to review your computation and please give me a
copy/print out of the details for my satisfaction.

We only have 1 computer in the personnel section intended for appointment preparation.
To expedite the process, we need additional set of computer for the In-Charge of the Secondary,
for us to simultaneously prepare their appointments. We cannot expect our requisition at this
moment believing that purchases of office equipment is a long procedure.

I appreciated your Office very much for acting on my query why I still have an
outstanding loan balance for my enchanced salary loan which was already paid up last October
2005, per print out of my monthly installments hereto attached.
My loan was granted last August 2001 and the first monthly installment started three
months after the release of such loan.
In your letter dated September 3, 2011, the outstanding balance of P38,401.19 is due to
interest on delayed loan payments and underpayments, but may I know how much interest do
you charge for three months of delay from September to November, 2001 and underpayment of
about P38.90 a month that it totaled to an amount almost equal to the sum of the loan granted.
How about my payments of P51,842.73 that I religiously pay continuously without gap
on due dates. Is it not credited promptly to my payments that it accumulated a very huge
interest? It is unfair on my part to pay interest/surcharges of delayed remittances/posting which
is not on my fault. Please Ma’am, I plea to review and recompute my liabilities so I can settle it
the soonest possible time to avoid additional burden for an ordinary employee like me with a
meager salary.

This is relative to the 1st indorsement from your office dated May 22, 2012 regarding the
application for transfer of your teacher, Ms. Elsa M. Amorio to our Division.
Please be informed that as of now we have no vacant Item. We can only hire her if you
will accommodate Ms. Lilibeth Amoguis, Teacher I in our Division who also wanted to transfer
to one of your schools. In fact, I am aware that they are having a swapping agreement but we
discouraged the method because our Office is strictly observing the rules and regulations of the
Civil Service Commission for personnel actions. As the case maybe, we have to publish the
vacant Item of Mrs. Amoguis for fifteen (15) calendar days, posting of the publication in three
(3) conspicuous places for another ten (10) calendar days and a request from the regional office
for authority to fill.
The process will take almost two (2) months long after their last day of service. There
will be a possibility of a gap of their services, but we will try to give priority on this and Mrs.
Amoguis is agreeable with this arrangement.
Please inform us if Mrs. Elsa Amorio is amenable with this situation and we will release
Mrs. Amoguis so we can hire Mrs. Amorio.

Madam:
This has reference to the application for retirement of the late Visitacion A. Lagudas filed
with this office effective April 1, 2010. As she was then very sickly, she filed her retirement
effective April 1, 2010 upon knowing that she turned 60 years of age on July 2, 2009 per live
birth with the NSO which was July 2, 1949 and not July 2, 1950 as what was reflected in all her
records.
You can observe that the application was dated March 30, 2010 and she was using the old
GSIS retirement form. Below of the application is her authentic signature. Lower right below is
the date her documents were processed.
Only this year that Maricel A. Lagudas, her only survivor submitted all other documents
required if a member died, in addition to the application previously filed by the late Visitacion
Lagudas. This was because she could not secure the death certificate of her mother unless all
indebtedness in the hospital be settled.
A discrepancy on the date of her death was found by your office in a certification made
by the school where she was formerly assigned, issued inadvertently, which caused the doubt in
your part as to the authenticity of the claim.
Please let this office rectify the lapse we have made and we appeal that the benefits due
her be released.

CERTIFICATION
This is to certify that per records of this Office, MRS. ANTIOQUIA B. CORDOVA ,
was employed by this Division as Regular Permanent Classroom Teacher effective November
12, 1973, as transferee from the Division of Zamboanga del Sur.
This is to certify further that she transferred back to the Division of Zamboanga del Sur
on September 25, 1982. A copy of the corrected service records is also attached to rectify the
service records previously issued reflecting May 31, 1976 as the effectivity of her appointment
as Regular Permanent instead of November 12, 1973 which is retroactive. The word cancelled
on her appointment superimposed the attestation of the CSC as provisional status upon
confirmation of her eligibility.
This certification is issued upon request of Mrs. Cordova to support her claim for GSIS
Retirement benefits.

I am very grateful that finally I have my disability retirement computation, but please
may I ask you to recompute my lumpsum so I may not pay the amount of P63,960.35 as deficit.
As I know, I am entitled to my monthly pension effective right after my disability, so I am entitled
to my monthly pension from December 2011 to June 2012. If so, I will only have a deficit of
P6,367.57.
Reflected also in my CSV claim is the total amount of P262,793.44, which amount is
already deducted from my 18 month lumpsum, in this case, I may have a net proceeds of my CSV
in the amount of P55,791.65.
As you may know, I still have to maintain my medications and I appeal that the deficit of
P6,367.57 be charged against the net proceeds of my CSV claim. Hopefully, I will be receiving
my pension on July, 2012 and thereafter.

This has reference to the Regular Permanent Re-Appointments per EO 366 of Dario
Ybañez , and Illuminada G. AraoArao of Pusok National High School and Ronald D.

Your office have disapproved their appointments for reasons that no record matched their
identity per your verification with the PRC.
We regret to inform you that in our dialogue with the concerned teachers we found out
that they were not able to renew their license with the PRC, which in my opinion was the cause
of the cancellation of their names from the roster of licensed professionals.
With our instruction, they went to the PRC to request for renewal of their license, but
adversely, they were given two to three months time pending final verification with the PRC
Manila, before the Commission can reissue their licensure.
This office therefore respectfully appeals to your good office to defer the finality of the
action taken on subject appointments until such time that their license will be released by the
Professional Regulation Commission ( PRC).

NOSCA of reclassified Items and retain Item No. TCH1-526111-1998.

With the implementation of the K-12 Basic Education Curriculum, this office identifies
the shortage of school teachers to handle the classes. DepED Central Office has provided this
Division 410 teacher items to be filled up before May 15, 2013 ( please see attached memo and
NOSCA). Said memo is setting a deadline to submit the List of Teachers to occupy the total
allocation of this Division, one half of it, identified from the Old Registry of Qualified
Applicants and the other half from the new RQA. Considering the COMELEC Election Ban on
hiring/assigning of public school teachers and employees , this office may not be able to meet the
deadline set by the Central Office.
In line with this, we are hereby requesting your good office to grant DepED Lapu-Lapu
City an exemption from said COMELEC Ban, and be allowed to hire from the old ranking, One

Sir:
Eight long years that our Department managed to run the office effectively with least
manpower, in compliance with E0 No. 366 and its implementing rules and regulations, dated
June, 2005, specifically the moratorium on the filling up of regular permanent itemized positions
vacated by incumbents who retired/transferred.
This year, our security guards occupying Regular Plantilla Items retired, effective
December 1, 2012 and July 1, 2013 respectively. The urgency of the services of security guards
cannot wait for the approval of the rationalization plan of this agency, wherein, we can hire and
fill up existing vacancies.
Inasmuch as the position is relevant for the security of the office properties and its
vicinity, we are requesting the authority to fill up said place of duty for the betterment of the
service.
This office elevates to you the condition of one teacher in our Division assigned in Sta.
Rosa National High School in the person of Joeffrey C. Sison. He has violated the leave laws,
rules and regulations that will cause him dropped from the rolls, but for humanitarian reasons,
we are deferring our actions because after his resignation dated April 7, 2013, he wrote a letter to
retract his pronouncement and asked to reinstate.
The reason for his absences is sickness and it maybe considered, had he informed the
immediate head of office his circumstances. In his letter, he admitted his lapses and plead for a
second chance.

This is relative to the appointment of Maria Belfe Plaza Jamera in this Division dated
June 2, 2014.
Please be informed that she is a transferee from the Division of Surigao City, Caraga
Administrative Region. Her last day of service in her previous agency was on June 17, 2014 and
her first day of service in this Division is June 18, 2014, the same date that should have been
reflected on the face of her appointment.
Her appointment was mixed up with the appointments of the newly hired teachers in SY
2013-2014 . We apologize the inconsistency of the date of her appointment which was similarly
dated with that of the newly hired on June 2, 2014.
It is in this context that this office requests your kind consideration to rectify the
effectivity of her appointment from June 2, 2014 to June 18, 2014.

Madam:
This has reference to the Regular Permanent Re-Appointments per EO 366 of the
following elementary schools teachers of this Division, namely:
Your office have disapproved their appointments for reasons that no record matched their
identity per your verification with the PRC.
We regret to inform you that in our dialogue with the concerned teachers we found out
that they were not able to renew their license with the PRC, which in my opinion was the cause
of the cancellation of their names from the roster of licensed professionals.
With our instruction, they went to the PRC to request for renewal of their license, but
adversely, they were given two to three months time pending final verification with the PRC
Manila, before the Commission can reissue their licensure.
This office therefore respectfully appeals to your good office to defer the finality of the
action taken on subject appointments until such time that their license will be released by the
Professional Regulation Commission ( PRC).

The undersigned would like to request for change of work schedule from 8:00 am to 5:00
pm. to a new schedule from 9:00 am. to 6:00 pm. effective upon your approval.
This is in consideration with the traffic in our place at Basak, Lapu-Lapu City in going
to and from our office which is consistent daily and besides of going home late at past six p.m.

Consistent with the provisions of the CSC Omnibus Rules on Appointments and other
personnel Actions, this office hereby prescribes the following guidelines ( see enclosure) to
govern the processing of appointments to vacant positions and anticipated vacancies as a result
of chain promotion. This is done to ensure that said vacancies shall be filled up expeditiously.
You are hereby requested to coordinate with retirable employees (aged 60 and above) in
your respective schools and advice the latter to file their retirement papers at least one month
prior to the effectivity date of retirement. The employee concerned shall consider his/her
decision to retire final, once he/she submits his/her documents.

The undersigned was a Master Teacher formerly assigned in Bankal Elementary School
who retired from the government service effective January 1, 2016. My application has been
forwarded to the GSIS through DepEd Regional Office which was withheld by the same,
pending submission of the report of investigations regarding a case filed by the Ombudsman’s
Office against the undersigned for unpaid debts which had already been settled on November
14, 2009 per certification signed by Ricardo G. Lagunda, Equicredit Operation Head.
In this regard the undersigned respectfully request your kind office to issue a clearance
that she has been cleared with Equicredit based on the documents hereto attached.
