Your ward_____________________of class/sec________has been selected for

Chairman’s Cup 2022-23 and will represent the School Football Team on 14th and 15th
December at DPSG, Palam Vihar, Gurugram. School will provide the bus facility to the
students for the event. Parents are requested to drop their ward in the school by 7:15am on
December 14th, 2022 and pick up their ward from school after the event. Please ensure
COVID-19 protocol is maintained.
NOTE: Any further information regarding the schedule will be informed through
whatsapp group.
If you would like your child to participate in this event, please complete, sign, and return
the following statement of consent and release of liability by December 13th, 2022

I understand that this event will take place away from Delhi Public School Ghaziabad
and will be on a different place on stated dates. I further consent to the conditions stated
above on participation in this event, including the method of transportation. I also
indemnify Delhi Public School Ghaziabad for any unforeseen circumstances during the
tournament and travel.
