    Ma’am,
    Greetings of peace and joy!

    This letter intends to ask permission from your good office to allow me to have an advance
    maternity leave from June 20-June 30, 2022.
    My expected due date to undergo cesarean operation will be on July 10, 2022. Upon the
    recommendation of my doctor/OB-GYNE Dr. FLORENCE FLORES and DR. SHIRLEY YAP. I
    should have 2 weeks in advance to schedule in the hospital. I will also undergo swab testing on
    June 26, 2022 and I was also advised to stay at home while waiting for the result.
    Reporting to school while waiting for my delivery schedule is at risk on my part. With this I
    would like to have an advance leave starting June 20,2022- June 30, 2022 and hoping for you to
    consider it as a work from home on the said date.
    Attached herewith are my medical Certificate and other documents.
