Shaft Pre-Sink Concrete Liner Pouring Procedure

1.0 PURPOSE
This Standard Operation Procedure provides the information and instruction for pouring
cement from the Galloway deck, pre-sink operations
2.0 SCOPE
This procedure applies to Fruta del Norte (FDN) Vent Shaft Galloway Deck operations, sinking
of the shaft.

3.0 DEFINITIONS

4.0 RESPONSIBILITIES
The DC-DC-Thyssen Mining Shift Supervisor and the DC-DC-Thyssen Mining Project Manager
or Designate are responsible for implementing and enforcing this procedure. All DC-DC-
Thyssen Mining personnel are to follow this procedure.

5.0 PROCESS
5.1 Set up crane #2 in proper position and coordinate with crane #1.
5.2 Ensure all required PPE and 5-point harnesses are in good condition.
5.3 Ensure positive radio communication between both cranes and miners.
5.4 Ensure surface spotter on surface to coordinate movement of cranes and mud
truck.
5.5 Miners conducting the pour operations will transition on to Galloway deck and
climb down to 2nd deck. Miners will ensure required tie-off if handrails or chains are
not in place.
5.6 When moving Galloway there must be a spotter on the lower deck to ensure that
there is proper clearance between the Galloway deck and the shaft perimeter, curb
ring, etc. before and during the Galloway moving process.
5.7 Spotter will ensure first deck of Galloway is in the proper position.

5.8 Spotter will signal crane #2 to lift mud bucket designated position just above floor
of Galloway deck #1 and advise miners on deck #2 that the bucket is in position
and ready to pour.
5.9 Miners on deck #2 will instruct spotter when to open bucket doors to begin pour.
5.10 When bucket is empty, miners on 2nd deck will instruct spotter to close bucket
doors.
5.11 Spotter can then guide mud truck to proper position to refill the bucket
