1 This "Request for Information" form is basically a communication initiated by the Contractor.

This is accomplished whenever there is a need for some clarification that the Contractor wants as far as
the execution of construction procedures or any matter which they think there is a need for clarification.

3 All the queries of the Contractor shall be stated under the space provided for "Question/s".
4 The works affected with such query as well as the basis made or the reference drawings and/or specs for
the clarifications asked shall be declared under the spaces provided for "Works Affected" and
"Drawings/Specs Reference".
5 The PPMIU Engineer shall be the one to answer the inquiries made by the Contractor. The response
must be cleared with the Project.
