ASSOCIATION, INC. with principal address at Purok Roadside 2, Barangay Alijis,
Bacolod City;

That the list of the members and officers with their address and contact number is
hereto attached;

That the territorial jurisdiction of INNERVILLE HOMEOWNERS ASSOCIATION, INC.


is located at Purok Roadside 2, Barangay Alijis, Bacolod City, with a total area of 4,500
square meters (Proposed CMP Area: 4,500 sq.m.) as stated in Photocopy of Original
Owner’s Duplicate TCT No. 319935 registered in the names of Bondicia M. Peña m/to Suklin
Peña and classified as Residential as per Zoning Certificate issued by Mr. Frances Arañez,
Zoning Officer IV of Land Use & Zoning Division of City Planning and Development Office;

That Innerville HOA, Inc. is an on-site project based on the Barangay ISF Certification
as issued by Punong Barangay Deogracia B. De La Vega last October 17, 2022, certifying that
all 51 MBs are actual resident of the jurisdiction of the proposed site despite the
noncompliance of the 85% requirement occupancy rate (68.63%) that was observed during the
initial site visit conducted last September 25, 2022;

That there is no other existing homeowners association within the territorial jurisdiction
of INNERVILLE HOMEOWNERS ASSOCIATION, INC.;

That this certification is being issued upon the request of Ms. Dangeline T. Mendez
for its registration with Department of Human Settlement and Urban Development
(DHSUD).
