Koinonia Funeral Homes, a duly registered funeral service provider pursuant to the
provisions of Act 3883, as amended by Act 4147 and Republic Act No.863, and in
compliance with the applicable rules and regulations prescribe by the Department of Trade
and Industry BN No. 1264604 dated March 25, 2021 in the Philippines with Main Office,
Khan Commercial Arcade, Sibulan, Negros Oriental, represented by the undersigned.

We are offering direct service and lay-away program. Our lay-away programs geared
toward helping individuals and families who would want to prepare the most inevitable of
life, death, at the most reasonable prices at a short term.

We are currently in negotiation with DIOSDADA QUINONES and AQUILINA QUINONES

for the property described under Certificate of Original Title No. OP 50463. The facility
will be used as embalming facility to service clients in your barangay, town and
neighboring municipalities.

In this regard, we would like to request an audience with the Barangay Council and further
request that a public hearing be conducted for this project. As part of the requirements for
our application for building permit.
