I am Mary Louise B. Portugal, an OSGP Grantee rendering her service in the
Cash Management Office (a.k.a. Cashier’s Office). I am writing this letter in regards to my excused absences last December 6 (Half day) – December 8, 2022 due to my father being rushed to the ER of Philippine Heart Center for his unexpected angioplasty operation in the afternoon of December 6. He was able to admit himself at the Ospital ng Maynila that morning when he was feeling unwell and determined he has to undergo the said operation upon checking him up via the ECG. As the eldest sibling in the family, I was needed by my mother to take care of my youngest sister with Down syndrome while she was taking care of my father at PHC until his discharge (December 10).
I am hoping for your kind consideration regarding this matter.
